51 years F. Last period - 4 months ago. Pitta, Kapha prakriti , Vata aggravation now. 1. Has been experiencing numbness in toes and fingers when staying dormant or when sleeping. She used to have some numbness on and off in fingers in the last 2 years. But last month, she experienced this for 10 days every day while sleeping. But after 3 weeks, symptoms are appearing again. Not sure if this may be related to dropping estrogen levels based on the time of the month. She took Shatavari 3/4 tsp everyday for a few days last time and coincidentally after a few days, the numbness went away. Not sure if it was due to shatavari's effect. Her HBA1c is around 5.7 and her TSH is slightly elevated now (5.7) but T3 is normal. 2. She has a BM in the morning 80% of the time- but there is a tendency to skip a day now and then. She may have slight ama. Good eating habits ( rice, dhal, vegetables, buttermilk, sabzis, chappati, oatmeal, poha, fruits). Sleeps by 10.30 and gets up at 6.15. But mind tends to overthink or feel pitiful if something has disturbed her and she cant sleep immediately. Fairly active with yoga and walking. Not obese 3. Blood work is showing a slight elevation in ANA titer I want to introduce gentle home cleansing for ama, improve circulation for numbness, provide hormonal support during transition, make BMS regular, first thing in the morning. Are these good for a month regime? 1.Daily virechan with 1/2 tsp avipati before meals and 1/2 tsp triphala after meals - but i would like know if any other virechan can be recommded. 2. Ama pachana with musta and haritaki or even panchaloka churna ( need more guidance on how to use it) 3. Arjunasavam for circulation 4. Raisins, prunes, chia seeds are part of morning diet. But not fully helping with BM regularity 5. Guduchi for ANA tier 6. Sarawathi Arishtam for overthinking / self pity 7. Shatavari with warm milk at night

this appears mainly to be a peri-menopausal Vata aggravation occurring on a background of Pitta-Kapha prakriti with mild ama accumulation. The numbness, irregular bowels, disturbed sleep, overthinking, and emotional sensitivity suggest Vata affecting the nervous system and colon during hormonal transition. Mild agni disturbance and slight ama may also be contributing to circulation and nerve symptoms. –Rx: 1) trifla cap 1 cap at bedtime with warm water 2) Musta Churnam ¼ tsp twice daily after food 3) Haritaki Churna ¼ tsp at bedtime 4) Guduchi Ghana Vati 1 tab twice daily after food 5) Shatavari Cap 1 cap with warm milk at bedtime 6) Saraswatarishta 15 ml with equal water after dinner —Pathya (Advice): Warm, freshly cooked food; avoid dry, cold, stale foods 1–2 tsp ghee daily Regular yoga + pranayama + walking Sesame oil foot massage at bedtime Maintain fixed sleep routine (before 10.30 pm)

Thanks for the suggestions. Here are my current symptoms: 1. Numbness in fingers when sleeping ( may happen in one hand but its not consistent) 2. Slight tingling in big toe on left foot felt during the day 3. Some varicose veins ( no pain ) 4. Last period was after 4months but lasted 10 to 12 days with heavy bleeding for 3 days in between and then it was moderate bleeding 5. Last bloodwork showed slightly elevated ANA tier 6. I have regular BM if I take Sukumara kwatham ( 2 tabs at lunch ) or Triphala ( 2 tabs at night). Otherwise, I may skip a day here and there. 7. There is a white coating in the back area of the tongue in the mornings. I am considering: 1. Guduchi Sattwa for 3 months for ANA 2. Musta 1/2 tsp with dry ginger before meals 3. Kaisoru guggulu ( 2 to 3 tabs a day) for varicose veins. 4. Dhanwantra tailam application for fingers and toes numbing 5. Shatavari gulam after lunch for hormonal support 6. Should I continue sukumara kwath for menopause transition 7. What about manjistadi kashayam instead of Kaishoru guguglu 8. Any Ashwagandha needed for numbness support ? Can you please review and give me a combined protocol for all these symptoms without overloading on medicines.

The picture still looks predominantly like Rajonivritti-kala Vata prakopa with mild ama and rakta-srotas involvement, but not severe enough to justify multiple Kashayams + Guggulus + Rasayanas simultaneously. —The numbness is still intermittent and positional (during sleep), which makes me think of Vata aggravation, posture-related nerve compression, cervical involvement, or B12 deficiency rather than a severe circulation disorder. –The heavy 10–12 day period after 4 months of amenorrhea is common in perimenopause, but if it recurs, gynecological evaluation (endometrial thickness, fibroid, etc.) is advisable. –White coating on the posterior tongue suggests mild ama is still present. –Regular bowel movement only with Sukumara/Triphala indicates Apana Vata has improved but is not yet fully stabilized. –Mild ANA positivity alone does not justify aggressive autoimmune treatment. -----My simplified combined protocol -------Morning Guduchi Satva 500 mg–1 g Warm water -------Before lunch and dinner Musta ½ tsp + pinch of Shunthi with warm water -------After lunch Shatavari Gulam 1 tsp -------After meals Sukumara Kashayam tablet (continue current effective dose) -------Bedtime Triphala only if bowels are not moving regularly with Sukumara alone –External Dhanwantharam Taila massage to hands and feet Sesame oil foot massage at night Gentle calf exercises and walking for varicose veins -----Follow-up investigations Because numbness persists, I would strongly consider: Vitamin B12 Vitamin D CBC Ferritin Repeat thyroid profile (TSH, Free T4) -In fact, B12 deficiency, borderline hypothyroidism (TSH 5.7), and perimenopause together can easily explain these symptoms. —avoid adding both Kaishora Guggulu and Manjishtadi Kashayam right now. Too many medicines may cloud the clinical picture. Give the current regimen another 4–6 weeks while monitoring numbness, bowel regularity, sleep, and any recurrence of prolonged bleeding.

During peri-menopause, declining estrogen often aggravates Vata, especially in the majja dhatu and nervous system. The numbness, disturbed sleep, variable bowels, anxiety, and dryness pattern strongly suggest Vata aggravation with mild ama accumulation. Mild thyroid imbalance and elevated ANA also indicate the need for a gentle, nourishing, anti-inflammatory approach rather than strong cleansing. Your current thinking is quite good. A softer Vata-Pitta balancing regimen will work better than aggressive virechana. Suggested Ayurvedic Approach 1. Gentle Daily Cleansing & Bowel Regulation Instead of strong purgation daily, use mild anulomana. Triphala 1 tsp with warm water at bedtime — continue regularly. If constipation persists: Add 1 tsp ghee in warm milk at night or Eranda taila (castor oil) 1–2 tsp once weekly only. Avoid taking both Avipattikar and Triphala in higher doses together daily initially, as excess cleansing may further aggravate Vata. 2. Ama Pachana Good option. Best gentle combination: Musta + Guduchi + Haritaki after meals in small doses. Panchakola churna can be used, but cautiously because it may increase Pitta if used continuously. Simple approach: Warm water sipping through the day Avoid cold foods, curd at night, excess raw salads, reheated food. 3. Numbness & Circulation This is more of a Vata in srotas / nerve dryness picture. Helpful measures: Daily abhyanga with warm sesame oil or Ksheerabala tailam before bath Gentle stretching/yoga Avoid sleeping with pressure on wrists/elbows B12 and Vitamin D should also be checked. Arjunasavam is acceptable, but for numbness and nerve nourishment: Ashwagandha + Shatavari combination is often more beneficial than Arjuna alone. 4. Hormonal Support Yes — Shatavari is very suitable here. Best way: 1 tsp Shatavari with warm milk at night Add a small pinch of nutmeg if sleep is disturbed. It supports: menopausal transition dryness mood stability sleep Vata-Pitta balance 5. Elevated ANA / Immune Modulation Guduchi is an excellent choice. It helps: mild autoimmune tendency inflammation Pitta balance immunity without overstimulation Can be taken morning empty stomach or after meals. 6. Emotional Overthinking / Sensitive Mind This is classic Prana Vata aggravation. Instead of only Saraswatarishtam: Brahmi tea at night Nasya with 2 drops warm ghee or Anu taila in nostrils Evening screen reduction Yoga nidra / alternate nostril breathing Saraswatarishtam may help in small doses after meals if digestion is good. Most Important Lifestyle Advice Menopause needs more: oiling grounding warmth nourishment routine and less: excessive detox fasting dryness overstimulation Simple 1-Month Regimen Morning: Warm water Guduchi Light yoga/walk After breakfast/lunch: Musta + Haritaki (small dose) Evening: Early light dinner Night: Triphala Shatavari with warm milk Foot massage with sesame oil Important Medical Note If numbness increases, becomes one-sided, causes weakness, severe pain, or persistent night symptoms, evaluation for: cervical spondylosis carpal tunnel syndrome B12 deficiency thyroid neuropathy prediabetes neuropathy should also be done alongside Ayurveda. Overall, this appears manageable with a gentle Vata-pacifying rasayana approach rather than strong shodhana.

1. Immune & ana support guduchi Sattva – core medicine for immunity + inflammation balance 2. Varicose veins + blood cleansing Manjishtadi Kashayam (prefer over Kaishore Guggulu) 3. Digestion + tongue coatingmusta + Dry Ginger before meals (light dose) 4. Numbness / tingling Dhanwantharam Tailam massage on hands & feet (night) 5. Hormonal support Shatavari gulam only if needed (not daily compulsory) 6. Gut regulation Sukumara Kwath OR Triphala (only one)

This looks mainly like peri-menopausal Vata aggravation with mild ama and possible early neuropathy. Also check Vitamin B12 and Vitamin D once, along with repeat thyroid after 2–3 months. Your plan is mostly good. Simple gentle regimen for 1 month Avipattikar Churna 1/2 tsp at bedtime with warm water instead of before meals Triphala Churna 1 tsp at bedtime only if BM still irregular. Avoid taking both together daily initially. Guduchi Satva 500 mg twice daily after food for ANA / immunity balance Shatavari Churna 1 tsp with warm milk at night for hormonal transition and Vata-Pitta balance Saraswatarishta 15 ml + equal water after dinner for overthinking and sleep Musta Churna 1/2 tsp before lunch and dinner for mild ama pachana For numbness Daily sesame oil foot massage before sleep Gentle stretching and walking Avoid sleeping directly under fan/AC Instead of strong virechana now, prefer mild daily anulomana only. Diet Warm cooked foods Reduce curd at night, excess oats, dry foods Add 1 tsp ghee daily Continue raisins/prunes; add soaked black raisins at night

Your plan is broadly thoughtful, but in menopause-related Vata aggravation with mild hypothyroid tendency, constipation + numbness + anxiety + ANA elevation, the approach should be very gentle and not over-cleansing. Excess “virechan” or multiple strong formulations together can actually worsen Vata and increase numbness, sleep disturbance, and fatigue. Key correction first This is mainly Vata imbalance + hormonal transition + mild neuropathic tendency, not a “toxicity-only” picture So priority = nourish + lubricate + regulate bowels + calm nerves About your current ideas 1. Virechan daily (Avipattikar before meals + Triphala after meals) Not advisable. This is too frequent and can dry the gut further → worsens constipation and numbness. Instead: Avipattikar can be used only if acidity is present Triphala is fine but only at night (not after meals) 2. Ama pachana (Musta / Haritaki / Panchalavan) Musta + Haritaki is OK but mild dosing only Panchalavan is more for digestion stimulation, not for long-term daily use in Vata dominance Better approach: Musta alone or with ginger in mild form is safer 3. Arjunarishta (Arjunasavam) Good for circulation support, but not primary for numbness Can be continued if heart palpitations or vascular weakness exists 4. BM irregularity (main issue here) Your current approach is too “fiber-heavy” for Vata constipation Better: Warm water on waking + 1 tsp ghee Soaked raisins + 1–2 figs (not excess chia daily) 5. ANA elevation + Guduchi Guduchi is fine but keep it low dose and monitored, especially in autoimmune tendency 6. Saraswatarishta for overthinking Good choice, suitable for menopausal anxiety + sleep disturbance 7. Shatavari with milk Yes, this is actually one of the most suitable supports here If numbness improved after it, it suggests neuro-hormonal support response Safer 1-month Ayurvedic approach Morning Warm water + 1 tsp ghee Ashwagandha capsule – 1 capsule Night Shatavari powder – 1 tsp with warm milk Saraswatarishta – 15 ml with equal water after dinner Triphala – 1 tablet only at bedtime (not multiple times/day) For circulation & numbness Dashmool taila gentle massage on feet/hands before sleep Warm foot soak 3–4 times/week Important clinical note Since there is: ANA elevation numbness recurrence borderline TSH It is important to rule out early autoimmune or thyroid-linked neuropathy + B12 deficiency if not already done. Bottom line Yes, Ayurveda can support this well, but: avoid over-detox / frequent purgation focus on nourishment + Vata calming + sleep + gentle circulation support Regards, Dr Raghuveer (Ayurvedacharya)

Hello Dr Raghuveer, I do not want to add too may medicines, but along with the current symptoms ( occassional numbness , slightly elevated ANA titer), I also want to address some varicose veins that have recently developed along with occasional itching in legs, A small area near the ankles looks darker - maybe from itching or some impure blood. I am not sure. I want to consider an effective protocol to address the issues with least medicines - balance vata, prevent auto -immune, rakta issues in veins, menopause smooth transition Issues are all mild to moderate - Varicose veins, numbing in fingers or toes, occasional itching in shin/ ankle, occasional constipation, some whitish coating in colon area of tongue, blood work showing slightly elevated TSH and ANA titer ( sub clinical), want to ensure smooth transition to menopause ( periods absent for last 4 months). Please review this and let me know. 1. Triphala 1 tab at night with warm water 2. Shatavari gulam after lunch ( 1 tsp) 3. Musta 1/4 tsp after lunch ( to address ama) 4. Ashwagandha - 1 cap in morning before breakfast 5. Guduchi - 1/2 tsp after lunch ( Is Guduchi sattwa better - why have you mentioned that Guduchi has to be taken with caution) - is Shamana vati better? 6. Kaisoru gugullu - 2 tabs after lunch - for possible vein inflammation / blood purification , or is Triphala Guggulu better? 7, Chandrapraba vati- 1 tab after dinner - for veins and female hormone 8. Will Sukumara kwatham help? 9. Massage with Dhanwantra or Ksheerabala 10. Yoga poses for circulation 11. Will it help to do a once a month ’ mild’ purging with Trivrit lehyam or Gandarva hastadi ? - is any snehana required and what quantity of herbs for purging ? - i want the simple approach. Thanks!

Your approach is thoughtful and already quite balanced. Since symptoms are mild–moderate, the goal should be gentle Vata-Pitta balance, circulation support, mild ama reduction, and nervous system stability — without overmedicating. A simpler and more sustainable combination would be better. * Triphala 1 tab at night – good to continue * Shatavari gulam 1 tsp after lunch – continue for menopause transition and dryness * Musta 1/4 tsp – can continue short term if tongue coating and heaviness are present * Ashwagandha 1 cap morning – continue only if it suits digestion and does not increase heat/irritability * Guduchi sattwa is gentler and cooling compared to regular Guduchi churna, so better in your case with ANA tendency and possible autoimmune background * Kaishore Guggulu is more suitable than Triphala Guggulu here because of mild inflammatory/vascular component and itching * Chandraprabha vati once daily is fine for metabolism and mild urinary-hormonal support * Sukumara kwatham can help constipation, Vata balance, and menopause transition gently — good option if bowels are not fully regular For oils: * Ksheerabala is better if numbness/dryness/Vata symptoms predominate * Dhanwantharam is better if stiffness and circulation issues are more noticeable Yoga, walking, calf movement exercises, and leg elevation will help varicose veins significantly. I would not suggest monthly purgation right now. With your relatively delicate balance and menopause transition, repeated purging may aggravate Vata if not done properly. Instead, mild bowel regulation with Triphala/Sukumara is safer and simpler. No strong snehana or aggressive cleansing is needed currently. Overall, keep the protocol light and consistent rather than adding many herbs together. Regards, Dr Raghuveer (Ayurvedacharya)

Hello, In the Ayurvedic view, this 51-year-old female is transitioning through Rajonivrutti (Menopause), a Vata-dominant stage of life. The numbness (Supti) and elevated ANA titer suggest Vata-Vyanu disturbance and Dhatu Shithilata (tissue depletion) due to declining hormones (Artava Kshaya). The presence of Ama (toxins) is hindering the Srotas (channels), leading to irregular bowel movements and circulatory blockages. The goal is to clear the channels (Srotasodhana), ground the Vata, and provide Rasayana (rejuvenative) support. Prescription : 1. Management of Numbness & Circulation The numbness is likely a combination of Vata aggravation and perimenopausal hormonal shifts. Arjunasavam is good for heart health, but for peripheral circulation and nerve strength, Dhanwantharam preparations are superior. Recommendation: Dhanwantharam Kashayam. Dose: 15 ml kashayam with 45 ml lukewarm water, twice daily before food. 2. Hormonal Support & Vata Balance Shatavari is excellent for cooling Pitta and supporting estrogen levels. Since she has a Kapha prakriti and slight Ama, taking it with milk is fine, but adding a touch of ginger can prevent heaviness. Recommendation: Shatavari Gulam, This is a traditional jaggery-based confection better suited for menopause than plain powder. Dose: 10 grams twice daily after food. 3. Ama Pachana & Regulating BM Instead of a heavy daily virechan with Avipattikar (which is more for Pitta/Acidity), a combination that addresses both Ama and Vata-type constipation is needed. Recommendation: Abhayarishtam. Dose (Abhayarishtam): 25 ml with equal water at bedtime to clear Ama and ensure morning regularity. 4. ANA Titer & Overthinking (Mental Health) Guduchi is the best choice for the ANA titer as it is an Amrita (immunomodulator). For the mind, Saraswatarishtam is the gold standard for menopausal anxiety and overthinking. Recommendation: Saraswatarishtam Dose: 20 ml with equal water after lunch. Guduchi: Can be taken as Samshamani Vati (2 tablets twice daily). Refining Your Proposed Month Regime Virechan: Swap the Avipattikar/Triphala combo for Manjishtadi Kwath if you want to focus on ANA/Blood, or stick to Triphala Churnam (1 tsp) with warm water at night for simpler bowel management. Panchakola Churna: Excellent for Ama. Use 1-2 grams mixed with the first morsel of food with a little ghee to kindle Agni without aggravating Vata. Numbness (External): Apply Dhanwantharam Thailam (mildly warmed) to fingers and toes before bath. This is often more effective for “dormant” numbness than internal medicine alone. Prescription summary : 1. Dhanwantharam Kashayam: 15 ml + 45 ml water, twice daily before food (for numbness). 2. Saraswatarishtam: 20 ml with water after food (for mind/hormones). 3. Abhayarishtam: 25 ml with water at bedtime (for Ama and BM). 4. Shatavari Gulam: 1 tsp twice daily (for hormonal support).

Any Ashwagandha needed for numbness support - Is Dhanwantra kashayam a better approach? - Will Ashwagandha aggravate ANA values Can you please review and give me a combined protocol for all these symptoms without overloading on medicines.

You’ve outlined a thoughtful plan, and I can see you’re aiming for a gentle, supportive regime during this menopausal transition with Vata aggravation, mild Ama, and circulation issues. Let me walk through your ideas step by step, highlighting strengths and where adjustments may help: 🌿 1. Virechana (Gentle Cleansing) -Avipattikar + Triphala: Both are mild, safe choices for daily use. -Instead of daily virechana (which can weaken Agni if overdone), consider alternate-day Triphala at night and Avipattikar only when acidity or heaviness is present. -Other gentle options: Trivrit lehya (small dose) or Castor oil in warm milk once weekly for deeper but controlled cleansing. 🔥 2. Ama Pachana -Musta + Haritaki: Good for digestion and light Ama. -Panchaloka churna is stronger; use cautiously, ideally under guidance. -Simpler daily support: Hingvastak churna (¼ tsp after meals) to reduce gas and Ama without over-cleansing. ❤️ 3. Circulation & Numbness -Arjunarishtam: Excellent for circulation and mild cardiac support. -Add warm sesame oil foot massage (Padabhyanga) before bed to improve peripheral circulation and reduce numbness. -Gentle yoga: Pawanmuktasana series and Viparita Karani help circulation. 🌸 4. Hormonal & Emotional Support -Shatavari with warm milk at night: Very appropriate for Vata aggravation and menopausal transition. -Sarawatarishtam: Can help with mood swings and overthinking, but keep dose low (5–10 ml with water). 🍇 5. Bowel Regularity -Raisins, prunes, chia are good, but if not sufficient: -Soak 5–7 black raisins overnight, eat in morning. -Warm water with ghee (½ tsp) at bedtime helps soften stools. -Avoid skipping meals; keep routine steady. 🧘 Lifestyle Anchors -Maintain early bedtime and wake-up (already good). -Add Nadi Shodhana pranayama before sleep to calm overthinking. -Keep meals warm, light, and freshly cooked. -Avoid excess raw foods and cold drinks (increase Vata).
