Bhallatak modak is a strong ushna and tikshna medicine and it is mainly useful in kapha dominant or chronic non bleeding piles It is generally not advised in active pittaj arsh or when there is burning pain and bleeding because it can aggravate pitta and worsen symptoms If already started it should not be used for more than seven to ten days and only under supervision For relief focus on softening stools cooling pitta and regulating bowels use ghee with warm milk at night drink adequate water avoid spicy oily food long sitting and straining Sitz bath with lukewarm water daily will help If pain and bleeding persist bhallatak modak should be stopped and safer pitta pacifying medicines should be preferred
