PGAD is mainly due to overactive pelvic floor muscles. You can focus on relaxation techniques. Mainly reverse kegel, child’s pose, slow breathing, belly breathing. Pavanamuktasana, cobra pose can also included. By these techniques for 2 weeks you didn’t get any relief start some internal medicine 1. Brahmi capsule 0-0-1 at night. 2. Triphala choornam 1/2 tsp at morning with warm water. Externally you can massage head with ksheerabala tailam, this oil will also helps to relieve stress. Take care, Dr. Shaniba

Your symptoms suggest a condition similar to PGAD (Persistent Genital Arousal Disorder), but the pattern you describe especially occurring during sleep, around ovulation, along with gas movement, fatigue, cravings, and mood changes often indicates a strong hormonal and nervous system involvement rather than only a genital problem. What may be happening in your body Hormonal fluctuations during ovulation Around ovulation, estrogen and other reproductive hormones rise and then fluctuate. These hormonal shifts can: Increase sensitivity in the genital nerves Trigger spontaneous arousal sensations or orgasm during sleep Affect mood, causing anxiety, sadness, or emotional sensitivity Nervous system overstimulation The clitoris and pelvic organs are supplied by very sensitive nerves. When the nervous system becomes hyper-responsive (sometimes due to stress, hormonal shifts, or pelvic congestion), it can create: Unwanted genital stimulation sensations Orgasm-like feelings during sleep Pelvic heaviness or pressure Digestive involvement (gas and upward air movement) The gassy feeling and upward movement of air after the episode suggests digestive imbalance. Gas accumulation in the abdomen can increase pelvic pressure and nerve stimulation, which may worsen these sensations. Sugar cravings and fatigue During ovulation some people experience: Increased sugar cravings Low energy and heaviness Sleepiness High sugar intake can lead to blood sugar spikes and crashes, which can worsen fatigue, mood swings, and hormonal imbalance. Skin dryness and weight gain around the waist These symptoms can occur when: Hormones fluctuate frequently Digestion and metabolism slow down Stress hormones increase Why it affects your mood Experiencing involuntary sexual sensations can be very distressing emotionally, especially when they occur during sleep or without desire. The repeated discomfort, bloating, and hormonal changes can contribute to: Depression Anxiety Low motivation Feeling emotionally drained Things that may help initially Some lifestyle changes can significantly reduce these symptoms: 1. Improve diet around ovulation Reduce refined sugar and heavy carbs Eat balanced meals with protein, vegetables, and healthy fats Avoid overeating sweets during that phase 2. Support digestion Eat lighter meals at night Avoid very late dinners Add ghee in your diet specially with your daals Warm foods and herbal teas with ginger fennel and carrom seeds for digestion can help reduce gas 3. Regulate sleep and stress Maintain a regular sleep schedule Gentle exercise like walking, yoga, or stretching can help calm pelvic nerves 4. Pelvic relaxation practices Deep breathing Pelvic floor relaxation exercises Avoid prolonged sitting… Now let me explain the ayurveda understanding and management. The symptoms you are describing suggest a Vata imbalance. In Ayurveda, when aggravated Vata moves upward instead of its natural downward direction, it can cause unusual genital sensations, gas, and discomfort. This condition is called Udavarta. A simple home remedy is to use warm sesame oil. Gently massage a small amount around the genital and lower abdominal area, and you may also place a small cotton piece soaked in sesame oil in the vagina for about 10 minutes daily to help calm Vata. If symptoms continue, it is best to consult an Ayurvedic doctor for proper treatment.
