Namaste, PGAD-like symptoms can sometimes be related to hormonal changes, pelvic nerve sensitivity, stress, and digestive imbalance. Since you notice it more during the ovulation phase with sweets cravings, gas, and mood changes, balancing hormones and digestion may help reduce discomfort. Simple supportive measures: Try to reduce excess sweets and refined carbohydrates, especially during ovulation. Eat warm, balanced meals with good protein, whole grains, and vegetables. Support digestion with cumin, fennel, or ginger tea after meals to reduce gas. Gentle yoga, pelvic relaxation exercises, and meditation may help calm the nervous system. Maintain regular sleep and manage stress, as emotional stress can worsen symptoms. Ayurvedic medicines that may support hormonal and nervous balance: Ashwagandha – traditionally used to support the nervous system and stress balance. Shatavari – commonly used to support female hormonal balance and reproductive health. Because PGAD can involve neurological and hormonal factors, it is important to also consult a gynecologist or neurologist for proper evaluation. A personalised plan combining medical guidance, diet, and supportive therapies is usually the best approach.

PGAD is mainly due to overactive pelvic floor muscles. You can focus on relaxation techniques. Mainly reverse kegel, child’s pose, slow breathing, belly breathing. Pavanamuktasana, cobra pose can also included. By these techniques for 2 weeks you didn’t get any relief start some internal medicine 1. Brahmi capsule 0-0-1 at night. 2. Triphala choornam 1/2 tsp at morning with warm water. Externally you can massage head with ksheerabala tailam, this oil will also helps to relieve stress. Take care, Dr. Shaniba

Your symptoms suggest a condition similar to PGAD (Persistent Genital Arousal Disorder), but the pattern you describe especially occurring during sleep, around ovulation, along with gas movement, fatigue, cravings, and mood changes often indicates a strong hormonal and nervous system involvement rather than only a genital problem. What may be happening in your body Hormonal fluctuations during ovulation Around ovulation, estrogen and other reproductive hormones rise and then fluctuate. These hormonal shifts can: Increase sensitivity in the genital nerves Trigger spontaneous arousal sensations or orgasm during sleep Affect mood, causing anxiety, sadness, or emotional sensitivity Nervous system overstimulation The clitoris and pelvic organs are supplied by very sensitive nerves. When the nervous system becomes hyper-responsive (sometimes due to stress, hormonal shifts, or pelvic congestion), it can create: Unwanted genital stimulation sensations Orgasm-like feelings during sleep Pelvic heaviness or pressure Digestive involvement (gas and upward air movement) The gassy feeling and upward movement of air after the episode suggests digestive imbalance. Gas accumulation in the abdomen can increase pelvic pressure and nerve stimulation, which may worsen these sensations. Sugar cravings and fatigue During ovulation some people experience: Increased sugar cravings Low energy and heaviness Sleepiness High sugar intake can lead to blood sugar spikes and crashes, which can worsen fatigue, mood swings, and hormonal imbalance. Skin dryness and weight gain around the waist These symptoms can occur when: Hormones fluctuate frequently Digestion and metabolism slow down Stress hormones increase Why it affects your mood Experiencing involuntary sexual sensations can be very distressing emotionally, especially when they occur during sleep or without desire. The repeated discomfort, bloating, and hormonal changes can contribute to: Depression Anxiety Low motivation Feeling emotionally drained Things that may help initially Some lifestyle changes can significantly reduce these symptoms: 1. Improve diet around ovulation Reduce refined sugar and heavy carbs Eat balanced meals with protein, vegetables, and healthy fats Avoid overeating sweets during that phase 2. Support digestion Eat lighter meals at night Avoid very late dinners Add ghee in your diet specially with your daals Warm foods and herbal teas with ginger fennel and carrom seeds for digestion can help reduce gas 3. Regulate sleep and stress Maintain a regular sleep schedule Gentle exercise like walking, yoga, or stretching can help calm pelvic nerves 4. Pelvic relaxation practices Deep breathing Pelvic floor relaxation exercises Avoid prolonged sitting… Now let me explain the ayurveda understanding and management. The symptoms you are describing suggest a Vata imbalance. In Ayurveda, when aggravated Vata moves upward instead of its natural downward direction, it can cause unusual genital sensations, gas, and discomfort. This condition is called Udavarta. A simple home remedy is to use warm sesame oil. Gently massage a small amount around the genital and lower abdominal area, and you may also place a small cotton piece soaked in sesame oil in the vagina for about 10 minutes daily to help calm Vata. If symptoms continue, it is best to consult an Ayurvedic doctor for proper treatment.

The symptoms you are describing show a connection between reproductive system nervous system digestion and hormonal rhythm in the body during the ovulation phase there is natural rise in heat heaviness and fluid movement in the body when food during this time becomes high in sweets and heavy carbohydrates digestion becomes slow and air movement inside abdomen increases this air can move upward creating gas pressure bloating and unusual sensations in pelvic region during sleep when the body is relaxed the nerves around clitoris and pelvic muscles can respond to this internal pressure and stimulation which can lead to orgasm like sensation without ejaculation The sadness heaviness sleepiness craving for sweets dryness in skin increase around waist and anxiety indicate disturbance in digestion metabolism hormonal rhythm and nervous system when digestion becomes weak internal gas accumulates and circulation of energy in pelvic region becomes irregular which can create discomfort during sleep and affect mood and energy during day In this situation the focus should be on restoring balance in digestion calming nervous system and reducing internal gas try to keep food simple warm and light especially around ovulation days reduce sweets refined flour bakery foods and heavy meals prefer freshly cooked vegetables light grains warm water through the day small amounts of ginger or cumin in food can support digestion regular sleep timing light walking after meals and simple breathing practices like anulom vilom and bhramari help settle the nervous system and reduce internal pressure in abdomen When digestion becomes stable and nervous system becomes calm hormonal rhythm slowly begins to regulate and the unusual sensations during sleep along with emotional heaviness and tiredness gradually reduce the body has the capacity to regain balance when daily routine food and mental state are supported in a consistent way patience and steady routine play an important role in recovery and improvement over time
