Doctors' responses

Hi! I’m an assistant to the Ask Ayurveda doctors. I’ve carefully reviewed your question.

I will explain your symptoms through the Ayurvedic perspective so you understand what is happening in your body as a whole.

Your lighter and shorter periods, along with longer cycles, insomnia, hair fall, fatigue, and weight gain, can be understood through an Ayurvedic lens as imbalances in the doshas, particularly Vata and Kapha.

1. Menstrual Changes: Lighter and shorter periods with longer cycles may indicate Vata imbalance, which can lead to irregularities and dryness. Vata, when aggravated, can disrupt the natural flow and rhythm of your menstrual cycle.

2. Insomnia: This is often linked to Vata dosha as well, which governs the nervous system. Excessive Vata can lead to restlessness and difficulty in maintaining a restful sleep.

3. Hair Fall and Fatigue: Hair fall may be a sign of both Vata and Pitta imbalances, while fatigue suggests a weakened Agni (digestive fire) and possible accumulation of Ama (toxins), leading to low energy and vitality.

4. Weight Gain Around the Belly: This is typically associated with Kapha dosha. Kapha imbalance can lead to sluggish metabolism and accumulation of excess fat, especially around the abdomen.

Ayurveda views these symptoms as interconnected, indicating a systemic imbalance rather than separate issues. It is crucial to address the root cause, which often involves digestion, lifestyle, and stress management.

To give doctors a precise and effective treatment plan, your answers to the questions below are essential. Without this information, the recommendations will be more general and less accurate.

Please answer each question as clearly as possible:

1. How would you describe your digestion? Do you experience heaviness or bloating after meals, and is your appetite strong or weak? 2. Are there any specific times when your insomnia worsens, such as after meals or during certain activities? 3. How severe is your hair fall? Does it increase during stress or with certain hair products? 4. Do you notice any specific triggers for your fatigue, like specific foods, activities, or times of day? 5. Is there a particular time of day when your weight gain or belly heaviness feels more pronounced? 6. How regular are your bowel movements, and do you notice any signs of Ama, such as a coated tongue or sluggishness? 7. Have you observed any changes in your mood or temperature sensitivity, such as feeling colder or experiencing mood swings?

Your detailed responses will help the doctors provide a more tailored and effective plan for your well-being.

●No, it is not as you think. Since you are suffering from PCOS, your periods may come later. I will give you some ayurvedic treatment. Start the medicines and you will get a good response. ●1/2 teaspoon dried ginger 1/2 teaspoon black pepper 1/2 teaspoon long pepper Method - 2 glasses of water Pour all ingredients into 2 glasses of water and boil until reduced by half. • Drink lukewarm morning and evening. Benefits - • Strengthens the digestive system and reduces Kapha dosha. Helps improve metabolism in PCOS. Balancing Kapha-Vata dosha in thyroid ●Mix equal amounts of ashwagandha, kapikacchu, and black sesame seeds and take 5 grams of the mixture with milk. This is beneficial for arthritis, weakness, and fatigue.Body Rejuvenator Cook 5 grams of Ashwagandha powder in 200 ml of milk and strain it until it is reduced to 100 ml. Consume lukewarm. It deeply revitalizes the body, providing strength, vitality, and stability. ●Asparagus is a major Ayurvedic chemical and tonic. Its main properties are: • Smooth, • Bile-soothing, • Sweet, • Cooling, • Vigor-boosting. For this reason, asparagus is considered superior in the Ksheerpak method. ●Shatavari powder – 1 part Milk – 8 parts Water – 8 parts (usual proportion for single medicine) ●Method of Preparation 1. At 28, transitioning into perimenopause while biologically possible (Primary Ovarian Insufficiency)is statistically rare. In many cases, lighter periods (Hypomenorrhea) and associated symptoms are more likely linked to hormonal imbalances, stress, or nutritional deficiencies that affect the Artava Vaha Srotas (the channels carrying menstrual fluid). The Ayurvedic Perspective In Ayurveda, the menstrual cycle is a reflection of your overall Rasa Dhatu (plasma/nutritional fluid) and the balance of Apana Vayu (the downward-moving energy). Rasa Kshaya (Nutritional Depletion): If the “Rasa” (first tissue layer) is depleted due to stress, poor diet, or over-exhaustion, the “Upadhatu” (secondary tissue), which is the menstrual blood (Artava), becomes scanty or light. Vata Dushti: An increase in the cold and dry qualities of Vata can “constrict” the channels. This leads to lighter flow, irregular timing, and symptoms like anxiety or sleep disturbances that can mimic perimenopause. Avarana (Obstruction): Sometimes, Kapha or Ama (toxins) can slightly obstruct the flow, leading to a “low volume” appearance, though Vata-dominant depletion is more common at this age. Suggested Management from Kerala Ayurveda The goal here is Artava Janana (promoting menstrual health) and Balya (strengthening the reproductive system). 1. Menwin Tablet This is a proprietary formulation from Kerala Ayurveda specifically designed for irregular or scanty periods. It acts as a uterine tonic that helps regulate the hypothalamic-pituitary-ovarian axis. Dose: 1 to 2 tablets twice daily after food, usually started 10 to 14 days before the expected date of the period and continued until the flow ends. 2. Sapthasaram Kwath (Tablet or Liquid) This is a classical formulation used for “Gulma” (abdominal masses/bloating) and “Rajadosha” (menstrual disorders). It is excellent for balancing Apana Vayu and relieving the pelvic discomfort that often accompanies lighter cycles. Dose (Liquid): 15 ml mixed with 45 ml of lukewarm water, taken twice daily on an empty stomach (about 30 minutes before food). Dose (Tablet): 2 tablets twice daily before food with warm water. 3. Ashwagandhadirishtam Since you are experiencing “other symptoms” (likely fatigue, mood swings, or sleep issues similar to perimenopause), this Arishta helps nourish the nervous system and manage stress-induced hormonal shifts. It acts as a “Rasayana” (rejuvenative). Dose: 20 ml mixed with an equal amount of lukewarm water, taken twice daily after meals. 4. Kumaryasava This is a fermented preparation of Aloe Vera, which is a potent “Artava-janana” (menstrual promoter). It improves the quality of the endometrial lining and supports liver function, which is crucial for hormone metabolism. Dose: 20 ml with equal water twice daily after food. Lifestyle and Diet Sneha Pana (Internal Oleation): Incorporate 1 teaspoon of A2 Cow Ghee or Shatavari Ghee in your morning routine to counter the “Rookshata” (dryness) of Vata. Abhyanga: Self-massage with warm Dhanwantharam Thailam on the lower abdomen and lower back for 10 minutes before a bath can help ground the Vata. Diet: Favor “Warm and Moist” over “Cold and Dry.” Include soaked raisins, dates, and sesame seeds (Til), which are traditionally known to improve menstrual flow.

Your condition is correlated to PCOS and insomnia. Not due to perimenopause. Hair fall, Fatigue - all indicate malabsorption of food and due to anxiety and stress. Keep regularising your sleep routine. Go to bed before 10pm and 8-hour sleep should be maintained. To help with insomnia Nasya and talam can be tried along with one internal medicine. 1. Nasya with ksheerabala tailam 2 drops in each nostril after a steam inhalation and abhyanga(with same tailam) at empty stomach in early morning. 2. Talam can be done at 5pm with ksheerabala. wipe off after 30 minutes. 3. Manasamitram vatakam 1 at night. Follow up after 2 weeks. Improvement in sleep quality will be visible after 2 weeks. For other issues. First we can focus on malabsorption. 1. Ashtachoornam 1/2 tsp with buttermilk at lunch time. THis will help in indigestion problems and bloating issues. 2. Asokarishtam 10ml twice daily just after food. It will helps in uterine problem. Just follow up after 2 weeks. Take care, Dr. Shaniba

My diet has been organic sattvic and so healthy for 3 years. Mung daal, cheela, quinoa, veggies such as spinach carrot Karela Loki bok choy beets, organic raw cow milk paneer. How can i still be having all these issues? The only thing is these last 3 years I’ve had terrible sleep. Waking between 12-3 am. Tell me some lifestyle suggestions for this too please

Diet is good Problem is bad sleep Why waking 12–3 am Pitta time → mind active What to do Sleep by 10 pm No phone at night Warm milk + little ghee 5 min deep breathing take Ashwagandha with milk

I hear you—and honestly, what you’re experiencing can feel confusing and a bit scary, especially with a history of PCOS. Let me reassure you first: this does not typically sound like perimenopause at 28. It’s much more likely that your PCOS + hormonal imbalance is driving these changes. Since your symptoms are moderate-to-severe and long-standing, you’ll need a structured 8–12 week correction plan. Hormone balancing + cycle correction 1)shatavari gulam – 1 tsp twice daily after meals 2)ashokarishta – 15 ml + equal water after lunch & dinner PCOS + metabolism correction 3)kanchanar guggulu – 2 tabs twice daily after meals 4)triphala tablets – 1–2 tabs at bedtime Stress + sleep + fatigue ashwagandha tablet – 1 tab at night with warm milk Your body is not shutting down—it’s asking for regulation and nourishment. With the right correction, periods can absolutely become healthier again, and fertility can be preserved. Please start the above Ayurvedic treatment regularly and observe the response. Follow-up after 2–3 weeks is very important so we can assess your progress and make any necessary adjustments in medicines or dosage. Take care

At 28 years, this is not perimenopause. These symptoms are more likely due to PCOS with hormonal imbalance. Lighter periods, longer cycles, hair fall, belly fat, and sleep issues are commonly seen when hormones are not balanced properly. Do this: Eat at fixed times, do not skip meals Avoid sugar, bakery, processed food Prefer home food: roti, rice, dal, vegetables Include seeds (flaxseed, pumpkin seeds) Add 30–40 minutes daily exercise (mix of walking + strength) Maintain proper sleep timing (10–10:30 PM) Avoid late nights (important for hormones) Drink warm water 1 tsp ghee daily Avoid excess tea/coffee Medicines: Kanchanar guggulu 2 tablets twice daily after food Ashokarishta 15 ml with equal water twice daily after food Shatavari churna 3 grams at night with milk Trikatu churna 1/2 tsp before food twice daily This will help regulate cycles, reduce hair fall, improve energy and hormonal balance. Also get these tests once: Thyroid profile Hb, Vitamin D, B12 With proper treatment, cycles can become regular again. For personalised plan and monitoring, you can consult me.
