HELLO DEEPIKA,

I’m truly sorry to hear about your mother’s condition- it’s very challenging to see a loved one in distress, especially when bedridden and unconscious.

When dealing with extreme diaper rash and skin breakdown in a patient who is unconscious and bedridden, the condition can worsen rapidly and lead to pressure ulcers or infections if not managed intensively.

URGENT MEDICAL POINTS Please understand that severe rashes with skin disruption are medical issue and need urgent wound care. While Ayurveda is helpful, modern wound management and infection control should not be ignored.

CAUSES IN THIS CASE -constant skin contact with urine and feces -lack of air circulation and friction -infrequent diaper changes -immobility leading to pressure injuries -poor hygiene due to unconscious state

DAILY ROUTINE CARE

1) FREQUENT DIAPER CHANGES -change every 1-2 hours or immediately after a bowel movement -even he best remedies will fail if the area remains in contact with stool

2) CLEANING AFTER EACH DIAPER CHANGE -use lukewarm water and a soft cloth or cotton pads -avoid chemical wipes. Use gentle, natural cleansers

3) AYURVEDIC CLEANSING AND HEALING PASTES apply the following after cleaning and drying the area

TRIPHALA DECOCTION WASH -make a decoction from triphala(haritaki, vibhitaki, amalaki) -wash the area with cooled decoction- antibacterial and healing

NEEM+ TURMERIC PASTE -mix neem powder+ turmeric powder in a little ghee(shatdhauta ghrita) -apply as a thin layer . Antibacterial, antifungal, and reduces inflammation -wash off after 15-20 minutes, then apply healing ointment

YASHTIMADHU PASTE -has skin soothing and anti-inflammatory properties -mix with honey or ghee and apply as a thin layer

4) HEALING OILS/OINTMENT Apply these 2-3 times a day, especially after cleaning

JATYADI TAILA -classical ayurvedic oil for wound healing and rashes -promotes tissue regeneration and reduces infection risk

KUMKUMADI TAILA for skin nourishment and gentle healing

COCONUT OIL WITH CAMPHOR -soothes, cool, and disinfects

AVOID commercial creams with steroids unless prescribed

5) DRYING AND AIR CIRCULATION -after applying oil, let the skin air dry for 5-10 minutes before putting on a new diaper - if possible, keep her without a diaper for 1-2 hours a day, laying on absorbent sheets with barriers to avoid soiling bed -use pure cotton cloth diapers or open the diaper briefly if skin is too moist

If she were conscious, internal ayurvedic medicines like

-GANDHAK RASAYANA= 1 tab twice daily after meals

-TRIPHALA CHURNA= 1 tsp at bedtime with warm water

-MAHAMANJISTHADI KASHAYA= 15 ml twice daily before meals with water

THANK YOU

DR. HEMANSHU MEHTA

HELLO DEEPIKA, I’m really sorry to hear about your mother’s condition. When someone is bedridden, unconscious, and using diapers daily—especially with frequent bowel movements—the skin becomes very vulnerable. Continuous contact with stool, moisture, heat, and friction leads to diaper dermatitis, and when prolonged, it turns into skin erosion, ulcers, or even infection.

Let’s help your mother by focusing on 3 things:

Protecting and healing the skin Controlling the loose stools (if possible) Maintaining daily hygiene and moisture balance

External Care (Highly Important & Urgent):

After Every Stool Episode:

Gently clean the area with lukewarm water + few drops of neem oil or Triphala decoction Do NOT use soap every time; it worsens dryness Pat dry fully — no moisture should be left

After drying:

Apply a thin layer of Yashtimadhu Ghrita (licorice ghee) or Jatyadi Ghrita (If these are not available, use plain virgin coconut oil until you get them)

If skin is broken or oozing:

Mix Shuddha Gandhak + turmeric + rose water into paste and apply twice daily Dust with Herbal Prickly Heat Powder containing Yashad bhasma (Zinc oxide) to prevent moisture trap Diaper Tips:

Use cloth diapers at home or breathable adult diapers Change diaper immediately after every motion Keep baby powder with zinc or neem-tulsi base to prevent fungal growth

Internal Medicines (To Control Bowel Movements)

If possible to administer through feeding tube or oral spoon feed:

Kutajarishta – 10 ml + 10 ml warm water after every episode of loose motion Nagaramla Churna – ½ tsp with warm water twice a day Mustharishtam – 15 ml + 15 ml warm water at bedtime

Pathya (If on oral diet/feeds):

Use rice kanji, moong dal water, pomegranate juice, buttermilk (if not contraindicated) Add a little ghee to soft rice/kanji feeds — supports gut & skin healing Avoid milk, bananas, raw fruits, heavy food, and sugar

Tests Can Be Done

CBC CRP (to check for any active infection) Electrolytes (if loose motions are frequent) Stool routine and culture Blood sugar (if healing is delayed)

You’re doing your best in a very difficult situation. Even though she’s unconscious, proper skin care and gut support can make her much more comfortable and prevent complications.

If you have any doubts, you can contact me. Take care, Regards, DR.Karthika

HELLO DEEPIKA,

In patient who are bedridden and unconscious , prolonged diaper use- especially when these is frequent passage of stool- can lead to severe irritation of the skin, known medically as Incontinence-associated dermatitis. It’s not only painful but can also increase the risk of infection if not managed properly

1) GENTLE CLEANSING AFTER EVERY SOILING -clean the affected area promptly after each bowel movement -use lukewarm. water mixed with neem decoction to wash the area. These are natural antiseptic and reduce inflammation -do not use strong soaps or chemical laden. wet wipes, as the may worsen the rash

2) THOUROUGH DRYING -after cleaning, carefully pat the skin dry with a clean, soft cloth - allow the area to remain exposed to air for a few minutes to ensure complete drying -a low speed fan or gentle air dryer can be used for this purpose if neded

3)APPLICATION OF AYURVEDIC OILS AND PASTES to support healing and reduce discomfort, apply soothing natural formulation -JATYADI TAILA= best for skin wound and ulcer - fresh Aloe vera gel= cooks and accelerated skin repair - yashtimadhu paste mixed with ghee- has anti inflammatory and calming properties - in case of intense burning or itching, chandanadi taila or manjisthadi taila may be beneficial

4) PREVENT DIAPERING PRACTICES -use high-quality diapers that are soft and breathable -diapers must be changed immediately after each soiling to prevent skin damage -if possible allow the patient to remain diaper free for short periods by placing protective underpads - apply a thin layer of barrier oil like virgin coconut oil, castor oil to the skin after each cleaning to protect against moisturise and irritate

INTERNAL AYURVEDIC MEDICNES

-GUDUCHI CAPSUELS= 1 cap in morning =promotes detoxification and skin health

-AVIPAATIKKAR CHURNA= 1 tsp before meals or half and hour -helps balance excess Pitta and supports digestion

-KAISHOR GUGGULU= 2 tabs twice daily after meals = often used to purify the blood and reduce skin inflammation

DO FOLLOW

HOPE THIS MIGHT BE HELPFUL

THANK YOU

To manage extreme diaper rashes, especially under delicate conditions like yours, there’s a blend of Ayurveda and practical hygiene measures that can be applied. First, while diapers are essential, try to ensure they are changed as promptly as possible when soiled. This frequent changing, though challenging, can prevent further irritation.

In terms of Ayurvedic management, applying soothing herbal pastes can be quite beneficial. A paste of turmeric and water mixed with a bit of organic coconut oil can reduce inflammation and promote healing. Turmeric, known in Ayurveda for its antiseptic and anti-inflammatory properties, helps soothe and heal the skin. After cleaning and drying the affected area gently, apply this paste and let it stay for about 10-15 minutes before carefully rinsing it off without scrubbing.

If it’s accessible, pure aloe vera gel can offer cooling relief and aid in healing. Aloe vera needs to be sourced fresh or in a product free from additives, to ensure it does not irritate the sensitive skin further.

Focus also on the internal balance. While your mother is unconscious, if possible and with consent, incorporating Triphala as a mild digestive aid could help regulate bowel movements. Triphala is a classical Ayurvedic formula that supports gut health, but please consult with a healthcare professional before administering any internal remedies.

Nutrition also plays a role indirectly, so try to keep her diet (or feed, if she is on a feeding tube) anti-inflammatory. Including cooling foods and avoiding excessively heating foods like spicy, fried, or sour items, helps mitigate Pitta imbalance, which is often linked with skin inflammation.

Finally, creating an environment where airflow helps the skin to breathe can be significant. Even a few minutes daily of exposure to air rather than continuous diaper use can make a difference, but this must be balanced with hygiene.

If the skin is severely disrupted or shows signs of infection, immediate medical attention is essential. Conditions like this sometimes go beyond what home remedies can manage safely.
