A pilonidal sinus is a small cyst or tunnel at the base of the spine, often caused by ingrown hairs, excessive sweating, poor hygiene, prolonged sitting, or friction. While some people are more prone due to coarse body hair, obesity, or deep gluteal clefts, lifestyle factors like tight clothing and lack of movement can also contribute. If left untreated, it can lead to chronic infections, abscess formation, and repeated flare-ups. In Ayurveda, a pilonidal sinus is considered a "Nadi Vrana" (sinus wound) caused by an imbalance in Pitta (inflammation) and Kapha (blocked channels). Herbal pastes (Lepa) made with turmeric, neem, and triphala help reduce infection and promote healing. Castor oil or aloe vera can be applied externally to soften the cyst and reduce swelling. Guggul and Manjistha are beneficial for detoxifying the blood and preventing recurrent infections. Some Ayurvedic practitioners also recommend Kshar Sutra therapy, a specialized thread treatment that helps drain and heal the sinus without conventional surgery.
