Pregnancy infertility, thyroid treatment - #27306
have thyroid and I am taking 25 mcg tablet. All my reports are normal. Uterus is normal with no cyst or fibroid, and tubes are open. My periods are regular with a 30-day cycle, but I am not getting pregnant. I don’t have PCOD. My AMH is 1.25. In follicular study, my egg size was 23×16. My husband had varicocele and underwent microsurgery 5 months ago. Before surgery, his sperm motility was 10%. Now it has slightly increased, but morphology is still low. I am 30 years old and my husband is 43. Doctors are suggesting IVF with ICSI. What should we do?

Doctors’ responses
“Thank you for sharing all your details. From your reports, I can see many positive factors — your thyroid is well-managed, uterus and tubes are healthy, cycles are regular, and your egg growth is good. These are strong foundations for pregnancy. 🙌
After varicocele surgery, sperm quality (motility and morphology) often takes 6–12 months to show full improvement. So, it is still early, and progress can continue. Many couples in a similar situation conceive naturally or with minimal support during this recovery window.
Your AMH shows you have a fair reserve, and the follicle size you mentioned indicates healthy ovulation. What’s the most important now is focusing on: ✔ Supporting sperm recovery and motility ✔ Enhancing egg quality and uterine receptivity ✔ Maintaining thyroid balance and cycle regularity
✨ IVF with ICSI is definitely an option, but it is usually considered if natural chances or mild supportive treatments are not working. Right now, you still have scope for improvement before directly stepping into IVF.
🌿 With the right guidance for both partners at this stage, the chances of conception can improve significantly — naturally or with minimal assistance.
What should we do, ma’am? Should we go for IVF now or wait longer? We have been married for 7 years, I am 30 years old and my husband is 43. We are confused whether to try naturally or proceed with IVF

Hello I can understand your concern but dont worry we are here to help you out
✅AYURVEDIC PERSPECTIVE
Your case shows- ✔️Thyroid dysfunction (though now controlled) but it slows metabolism amd ovulation energy ✔️Low AMH (1.25) - reduced ovarian reserve, but egg growth is happening (23×16 size seen) ✔️Male factor infertility (low sperm motility & morphology despite surgery)
➡️IVF-ICSI is suggested because both egg quality (low AMH) & sperm parameters are compromise.
Ayurveda can be used either: ✅Before IVF to improve egg quality and sperm health, increasing chances of success ✅As a natural attempt for 6–9 months before deciding on IVF
With proper TREATMENT and PANCHAKARMA it is possible visit near by Ayurvedic physician for EXAMINATION and
✅ AYURVEDIC PLAN OF TREATMENT-

HELLO RAK,
You are 30 years old and trying to conceive. here’s a simple breakdown of your situation
Your menstrual cycle= normal (every 30 days). This means your body is capable of ovulation
Ovarian reserve (AMH 1.25 ng/mL)= slightly lower than average. This means you may have fewer eggs available , so time is important
Follicle size= 23*16mm. usually an egg is considered mature around 18-24 mm. Your follicles are borderline mature
Uterus and tubes= normal. There is no blockage or structural problem
Thyroid= you take 25 mcg of levothyroxine ; your levels are normal, so thyroid is not a current problem.
Husband’s sperm= he had varicocele surgery . Motility (movement) has slightly improved but shape (morphology ) is still low. Low morphology makes natural conception difficult.
SUMMARY= you are ovulating and your reproductive organs are healthy, but the challenges are -slightly reduced ovarian reserve -male factor infertility (poor sperm shape)
DOCTOR’S SUGGESTION= IVF with ICSI (injecting one sperm directly onto one egg) is the most effective option because of male factor infertility
TREATMENT GOALS
PRIMARY GOALS -improve egg quality and quantity (ovarian health) -improve sperm quality, motility, and morphology -support uterus lining for implantation -balanc hormones, reduce stress, and improve overall reproductive health
SECONDARY GOALS -reduce inflammation and toxins in the body -strengthen general immunity and vitality -support IVF/ICSI success if pursued
AYURVEDIC TREATMENT Ayurveda treats infertility as Vandhyatwa and focuses on balancing vata, kapha, and pitta in reproductive tissues (artava or women, shukra for men)
Treatment involves internal medications, external therapies, lifestyle , diet and yoga/pranayam
INTERNAL MEDICATIONS OF YOU
1) PHAL GHRITA= 1 tsp with warm milk on empty stomach in morning for 3-6 months consistently =supports ovulation, nourishes uterus and endometrium balances vata
2) ASHOKARISHTA= 15 ml with equal water after meals twice daily for 3 months =regulates menstrual cycle, strengthens uterus reduces stress on reproductive system
3) DASHMOOLARISHTA= 20 ml with equal water after meals twice daily for 3 months =deeply nourishes reproductive tissues, reduces vata imbalance, improves strength post-menstruation
4) RAJAPRAVARTINI VATI (if ovulation is delayed or cycles irregular)= 1 tab twice daily from day 4-14 of your cycle (follicular phase) =promotes healthy ovulation and timely menstruation
5) STRI RASAYANA VATI (baidyanath)= 1 tab twice daily with milk for 3 months =general female reproductive tonic, improves egg health, hormonal balance, mood
FOR HUSBAND
1) CHYAWANPRASHA RASAYAN= 2 tsp with warm milk morning and night =builds Ojas, enhances sperm vitality , immunity, and overall stamina
2) SPERMAN (himalaya)= 2 tabs twice daily after meals for 3 months =specificlly improves sperm count, motility , morphology
3) SHILAJITWADI VATI (with gold- if available)= 1 tab twice daily with milk for 3 months =rejuvinates reproductive strength, testosterone support, improves sperm DNA integrity
4) SAPTAMRIT LAUH= 250 mg twice daily with honey and ghee for 2 months =reduces oxidative stress on sperm, supports eye and brain function, helps hormonal balance
5) KAPIKACCHU PAK= 1 tsp with milk at bedtime for 3 months =classical aphrodisiac , improves semen volume and quality
EXTERNAL THERAPIES
1) OIL MASSAGE with warm ksheerbala taila =improves blood circulation, relaxes muscles, and balance vata
2) Follow day hot water steam/bath =enhances circulation to pelvic organs
LIFESTYLE MODIFICATIONS -sleep 7-8 gourd, maintain regular sleep-wake cycle -avoid stress; practice meditation or mindful relaxation -moderate exercise : brisk walking, yoga or swimming -avoid smoking, alcohol, and excessive caffeine -avoid very cold, processed, or fried foods
YOGA ASANAS -baddha konasana= improves pelvic blood flow -supta baddha konasana= relaxes reproductive organs -setu bandhasana= strengthens uterus and lower abdomen -viparita karani= improves circulation to pelvic organs

DIET -warm, cooked meals, avoid raw, cold foods
INCLUDE -milk, ghee, almonds, walnuts, sesame seeds -dates, figs, pomegranate, seasonal fruits -whole grains : rice, oats, barley -Include foods rich in Zinc, Selenium, Vitamin E, Folate for sperm and egg health
HOME REMEDIES -warm sesame il abdominal massage once daily -herbal decoction with shatavari, ashwagandha, and gokshura -chywanprasha daily 1 tsp with warm milk -drink warm water throughout the day to improve digestion and circulation
INVESTIGATIONS
FOR YOU -repeat AMH and FSH (follicular day 2-3) -Thyroid profile -ultrasound for ovarian and follicular monitoring -progesterone level mid-luteal phase to confirm ovulation
FOR HUSBAND -repeat semen analysis after 3 months of said treatment -DNA fragmentation test if considering IVF/ICSI
ADVICES
NATURAL CONCEPTION= possible, but chances are lower due to male factor and borderline AMH
AYURVEDIC THERAPY= can improve egg quality, sperm health, educes stress, and increase chances of conception
IVF WITH ICSI= likely necessary for higher success, especially if male sperm morphology remains low
INTEGRATIVE APPROACH= combine ayurvedic treatment, lifestyle changes, diet, yoga and IVF for best outcomes
TIMELINE= start ayurveda and lifestyle modifications immediately for 3-6 months to optimise natural or assisted conception
DO FOLLOW
HOPE THIS MIGHT BE HELPFUL
THANK YOU

WISH YOU SPEEDY RECOVERY
HELLO RAK ,
You are 30 years old, with normal thyroid function on 25 mcg medication, and your menstrual cycles are regular. Your uterus, ovaries, and fallopian tubes are all normal. Your AMH is 1.25, which indicates a mildly reduced ovarian reserve. Your husband had varicocele, underwent microsurgery 5 months ago, and his sperm motility has improved slightly, but morphology is still low.
At this stage, the difficulty in conceiving is likely due to a combination of mildly reduced ovarian reserve and sperm morphology issues. IVF with ICSI is a modern medical option that can help bypass sperm-related fertilization challenges. Ayurveda cannot replace IVF but can support fertility naturally by improving your ovarian health, hormonal balance, and your husband’s sperm quality, which may enhance the success of any assisted reproductive treatment.
Treatment Plan:
1. Ama Pachana (First 3–5 days): Triphala Churna – 1 tsp with warm water at night, to cleanse the digestive system and improve metabolism.
2. Internal Medications (After Ama Pachana): Chyawanprash – 1 tsp twice daily, for general health and immunity. Ashokarishtam – 2 tsp after meals twice daily, to support hormonal balance and uterine health. Shilajit Vati – 250 mg once daily, to improve energy, vitality, and sperm quality. Kapikacchu (Mucuna Pruriens) Powder – for husband, 1 tsp with warm milk once daily, to improve sperm count and motility.
3. External Care: Gentle abdominal massage with warm sesame or coconut oil 2–3 times a week to improve pelvic circulation. Avoid stress, excessive heat, or tight clothing around the reproductive organs.
Dietary Guidance:
Include ghee, whole grains, lentils, fresh fruits, nuts, and seeds. Consume milk, yogurt, and protein-rich foods for reproductive tissue nourishment.
Avoid processed foods, excessive sugar, and deep-fried items. For the husband: include walnuts, almonds, sesame seeds, pumpkin seeds, and green leafy vegetables to support sperm health.
Investigations :
Repeat semen analysis after 3 months of treatment for your husband. Follicular monitoring during next cycles to check egg quality and growth.
Hormonal profile if recommended by your doctor. Bowel, Micturition, Appetite, Sleep: Check bowel regularity and urine output. Maintain a balanced diet to improve appetite. Ensure 7–8 hours of quality sleep.
If both of you follow this plan consistently for 3–4 months, it can improve your ovarian environment and your husband’s sperm quality, potentially increasing the chances of natural conception or IVF success. Ayurveda works gently and steadily, so consistency is key.
Warm regards, Dr. Karthika
Thank u dea doc

Considering your situation, integrating Ayurvedic principles alongside medical advice can be supportive. Since your thyroid levels are under control with 25 mcg medication and you have a regular menstrual cycle, major focus can be given to nourishment and optimizing reproductive health through lifestyle and diet adjustments as per Ayurveda.
On the Ayurvedic front, improving ‘Shukra Dhatu’ (reproductive tissue) vitality is key. Balance and enhance your internal system by including ingredients like Ashwagandha and Shatavari in your daily regime. These adaptogenic herbs are known to nurture reproductive tissue, and help in balancing hormones. Consuming one teaspoon of Ashwagandha powder with warm milk in the morning, and Shatavari powder with a similar quantity in the evening may be of benefit.
Additionaly, consider prioritizing a diet that increases ‘Ojas,’ which signifies overall vitality and immunity. Eat whole grains, nuts, ghee, and green leafy vegetables. Avoid excessive cold, raw or processed foods as they can dampen your ‘Agni’ (digestive fire). Give preference to foods that are warm and freshly cooked.
For your husband, post microsurgery, enhancing ‘Veerya’ (sperm health) will be crucial. He might do well incorporating Kapikacchu and Gokshura powders. One teaspoon of each, taken with water in the morning, can assist in supporting sperm quality. Additionally, reducing stress through practices like yoga and meditation may further support reproductive health.
Engage regularly in practices that calming the mind, like meditation or Pranayama, could also enhance overall well-being for both of you. Concerning medical steps like IVF with ICSI, these are appropriate options provided by modern fertility treatments for conditions such as low sperm motility and morphology. Ayurveda could complement these methods by optimizing your internal balance and vitality.
Should you choose to proceed with the IVF pathway, continue to adhere to both ayurvedic and medical guidance to maximize your conception chances and sustain health through this journey.
Considering the information you have shared, it seems you are dealing with unexplained infertility, and your husband’s sperm parameters might be contributing factors to this situation. Ayurvedic and Siddha perspectives focus on balancing the doshas - vata, pitta, and kapha - to support reproductive health. Both partners can benefit from addressing any imbalances and optimizing their overall well-being.
First, ensure your thyroid function remains stable while continuing with your prescribed medication. Thyroid imbalances can interfere with fertility, so staying in regular consultation with your healthcare provider is crucial. Reduce stress levels, as stress can exacerbate vata and impact reproductive health. Consider pranayama (breathing exercises) and meditation.
As your AMH is 1.25, it’s essential to support ovarian health. Consuming herbs like Shatavari and Ashwagandha can support reproductive function and balance hormones. Shatavari improves fertility by nourishing the reproductive organs, and Ashwagandha reduces stress and boosts vitality.
Your husband’s recent surgery indicates a need for recovery and enhancement of sperm health. Herbs like Gokshura (Tribulus terrestris) can improve sperm quality. Additionally, a diet rich in antioxidants — comprising fresh fruits, nuts, seeds, and green leafy vegetables — is beneficial for sperm health.
Incorporating sesame oil self-massage (abhyanga) can also help balance doshas and promote calmness. Both should focus on maintaining a warm, nourishing diet to stimulate agni (digestive/metabolic fire). Foods like ghee, whole grains, and warm soups can be supportive.
While Ayurveda provides holistic support, if the time is sensitive and you’ve already explored these natural options, IVF with ICSI may be prudent given medical advice and the age of your husband. Seeking a blend of conventional and traditional guidance often provides the best outcome, so keep in close communication with both your allopathic and Ayurvedic practitioners for a collaborative approach.
