Help with My Daughter's Menstrual Irregularities - #42149
Dear Doctor, My daughter, who is 16 years old, has been experiencing significant menstrual irregularities and prolonged bleeding. Her recent period pattern has been: 19–28 May 2–3 June 15–19 June 17–30 August 7–8 October From 15 October onwards – on-and-off persistent bleeding Since 15 October, she has been having 2 days of heavy flow, then 1 day without bleeding, followed by another day of bleeding, and this cycle continues. Whenever there is a gap, she passes large clots when the bleeding resumes and feels extremely tired. The flow is heavy overall, and the bleeding is not stopping. Kindly advise the next steps and the necessary tests or treatment.

This can be affecting the Rakta dhatu (blood tissue) and may require both immediate attention and long-term management. It would be prudent to seek a gynecologist’s evaluation as heavy bleeding, especially with large clots, could lead to anemia and fatigue, potentially needing urgent medical intervention.
In terms of immediate action, iron levels and hemoglobin count should be assessed to check for anemia. Hormonal imbalances (such as thyroid disorders or PCOS) might also be contributing factors. An ultrasound could provide insight into the uterine structure, ruling out fibroids or cysts.
For Ayurvedic intervention, decoctions like Ashokarishta are often suggested for regulating menstrual flow, consumed post meals approx. 20ml diluted in equal water twice a day. Complement this with Shatavari Kalpa, ideally 1 tsp mixed with warm milk at bedtime, helping restore hormonal balance and nourish the reproductive tissues.
Diet should focus on grounding and nourishing qualities to settle Vata anomalies and cooling foods to balance excessive Pitta. Encourage consumption of root vegetables like sweet potatoes, ghee, and almonds to enhance her Dhatu Agni. Avoid excessive spicy, processed and fried items as they may aggravate Pitta leading to more bleeding.
Controlling stress through yoga or meditation can be beneficial. Bhramari Pranayama could be effective in calming down the nervous system. Rest is vital, so ensure she gets adequate sleep as it supports the body’s natural healing processes.
