Doctors' responses

Hlo,

What this most likely is-

- Long-standing unilateral goiter (left thyroid swelling) for 15 years - Thyroid function tests (TSH, T3, T4) are normal → this is called euthyroid goiter

Symptoms present: - Voice change - Frequent mild cough These suggest pressure effect on nearby structures (laryngeal nerve / trachea), even if swallowing is still normal.

First important point (very important) 👉 Medicines (Ayurvedic or allopathic) usually CANNOT reduce a goiter that has been present for many years, especially: - If it is nodular - If it has structural enlargement, not hormonal So the goal is: - Prevent further growth - Assess risk - Decide if surgery is needed or not Tests you MUST do (if not already done) Please check if these are done — if not, they are essential: 1️⃣ Thyroid Ultrasound (USG Neck)- This will tell: - Size of the swelling - Solid or cystic - Single nodule or multinodular - Any suspicious features 👉 This is the most important test for you.

2️⃣ FNAC (Fine Needle Aspiration Cytology) - Needed if ultrasound shows a nodule, especially because: - It is long-standing - There is voice change FNAC does NOT mean cancer — it is done to rule it out safely.

3️⃣ ENT evaluation (Indirect laryngoscopy) Because you have: - Voice change - Chronic cough - This checks vocal cord movement and nerve pressure.

Can it be reduced to normal without surgery? Honest answer: ❌ Very unlikely after 15 years

When surgery is advised:? Voice change present ✔️ Pressure symptoms (cough) ✔️ Cosmetic concern Growth on follow-up Suspicious FNAC 👉 In such cases, hemithyroidectomy (removal of one lobe only) is often enough, not full thyroid removal.

If surgery is NOT immediately required If FNAC is benign and size is stable: - Regular 6–12 monthly ultrasound - Avoid unnecessary iodine excess - Control inflammation and oxidative stress - Supportive Ayurvedic approach (to prevent worsening, not shrink drastically) - These will NOT shrink a big old goiter, but can help stability: ✨✨✨✨✨✨✨✨✨ 🌿 Useful options 1) Kanchanar Guggulu 2 tablets twice daily after food

2) Varunadi Kashayam 15 ml + equal water, twice daily after meal

3) Triphala at night (for detox)

⚠️ Avoid self-medication if FNAC not done.

Diet advice (vegetarian) ✔️ Use iodized salt, but don’t overuse ✔️ Adequate protein (dal, paneer, curd) ✔️ Selenium-rich foods: nuts (2–3 Brazil nuts/week if available) ✔️ Cook cruciferous vegetables (cabbage, cauliflower) — don’t eat raw daily

❌ Avoid: - Excess soy - Repeated raw salads with cabbage/broccoli - About your cough

Your cough is likely due to: - Mechanical irritation / pressure - Mild reflux sometimes worsens it

Simple tips: - Don’t lie down immediately after meals - Avoid very spicy food at night - Warm water sips

My strong advice 👉 Do ultrasound + FNAC + ENT check first After that, decision becomes very clear and safe.

Based upon your history of a 15 year long thyroid swelling with normal TSH, T3, T4, with no pain, no swallowing difficulty, mild cough and voice change, this indicates , non toxic (euthyroid) goiter. In Ayurveda, this is usually due to Kapha predominant swelling with local Vata involvement in the throat region.

Below is a commonly followed Ayurvedic medicine which is, used to control size, improve local symptoms, and prevent further enlargement. Since your thyroid function is normal, medicines are kept mild, not aggressive.

Morning Kanchanar Guggulu 2 tablets Warm water with warm water twice daily after food This helps reduce glandular swelling and Kapha accumulation

Varunadi Kashayam – 15 ml Mixed with equal warm water twice daily after meals Helps lymphatic drainage and reduces firmness

Trikatu Churna – 1/4 teaspoon With honey or warm water after food Improves local metabolism and reduces throat heaviness

Yashtimadhu Churna – 1/2 teaspoon With warm water at morning Helps voice strain, throat irritation, and cough

Avoid cabbage, cauliflower, soy, excess curd, cold drinks Prefer warm vegetarian food with ginger, cumin, black pepper in small amounts

Since this is long standing, complete reduction may not be possible, but size reduction, voice improvement, and cough relief can be seen with 3 to 6 months of treatment. Please get an ultrasound if not done recently.

I have already started Kanchnar Guguloo since six days, a lot of improvement in size and voice. Will there b any side effects of it, please guide!

Good that with this short period of time you are noticing some improvement Do not worry, it’s a classical medicine used especially for hormonal imbalances N in your case it’s actually working at root level so without any fear you can continue it

Right Sir

THANK YOU FOR CONTACTING ASK AYURVEDA

A UNILATERAL THYROID SWELLING PRESENT FOR 15 YEARS WITH NORMAL TSH, T3 AND T4 INDICATES A GOITER /THYROID NODULE. WHICH MEANS MEANS HORMONE FUNCTION IS NORMAL, BUT THE GLAND STRUCTURE HAS CHANGED. VOICE CHANGE AND FREQUENT COUGH SUGGEST THAT LOCAL IRRITATION NERVES OR AIRWAY.

IN AYURVEDA, THIS CONDITION IS SEEN AS GALAGANDA, CAUSED BY KAPHA DOMINANCE WITH LONG STANDING STAGNATION, MUCUS ACCUMULATION, AND TISSUE THICKENING.

AFTER 15 YEARS, MEDICINES ALONE MAY NOT MAKE IT COMPLETELY DISAPPEAR, BUT THEY CAN REDUCE SIZE, PREVENT FURTHER GROWTH, IMPROVE VOICE AND COUGH, AND AVOID SURGERY .

AYURVEDIC MANAGEMENT- KANCHANAR GUGGULU – One tablet twice daily after food with warm water

PUNARNAVADI KWATH-15 ML WITH EQUAL AMOUNT OF WATER TWICE DAILY AFTER MEALS, TO REDUCE LOCAL SWELLING AND FLUID STAGNATION

TANKAN BHASMA A PINCH WITH HONEY ONCE DAILY - HARD GOITERS AND VOICE ISSUES YASTHIMADHU CHURNA- half teaspoon with warm water once daily

AVOID COLD FOODS, CURD AT NIGHT, SOY, EXCESS RAW CABBAGE, CAULIFLOWER, MILLETS IN EXCESS AVOID COLD WATER AND REFRIGERATED FOODS INCLUDE WARM, LIGHT, COOKED MEALS USE TURMERIC, GINGER, BLACK PEPPER, JEERA AVOID SWEETS AND BAKERY ITEMS

DO NOT TAKE EXTRA IODINE SUPPLEMENTS UNLESS ADVISED. NORMAL THYROID TESTS MEAN DEFICIENCY IS NOT THETE .

INVESTIGATIONS -IF NOT DONE RECENTLY ULTRASOUND OF THYROID – ENT CHECK FOR VOICE CHANGE -LARYNGEAL NERVE FUNCTION

IF SIZE IS INCREASING RAPIDLY IF THERE IS BREATHING OR SWALLOWING DIFFICULTY IF FNAC SHOWS SUSPICIOUS CHANGES THEN OPT FOR SURGERY

YOUR NORMAL HORMONE LEVELS ARE A VERY GOOD SIGN. THIS IS A STRUCTURAL ISSUE, NOT A DANGEROUS HORMONAL DISEASE. WITH PROPER AYURVEDIC TREATMENT FOR 3 TO 6 MONTHS, YOU MAY NOTICE REDUCTION IN TIGHTNESS, COUGH, AND VOICE STRAIN, AND IN SOME CASES, PARTIAL SIZE REDUCTION US SEEN

Thanks, Sir

Hello, Your history of long-standing left-sided thyroid swelling (goiter) for about 15 years, with normal TSH, T3, and T4, along with voice change and frequent mild cough, but no pain or difficulty in swallowing, suggests a chronic, non-toxic (euthyroid) goiter.

This usually happens due to local gland enlargement, nodular changes, or long-term tissue hypertrophy, rather than hormone imbalance. The cough and voice change indicate local pressure or irritation of nearby structures, even though hormone levels are normal.

The treatment focus is on reducing gland size gradually, improving local circulation and lymphatic drainage, preventing further enlargement, and relieving cough and voice strain.

🔎 Recommended Investigations (If not done in the last 6–12 months) 1.Ultrasound neck (thyroid) – very important Size of gland Presence of nodules Solid vs cystic nature 2.Thyroid Profile (TSH, T3, T4) – baseline (already normal) 3.Anti-TPO antibodies – to rule out autoimmune thyroiditis 4.FNAC – only if ultrasound shows suspicious or large nodules These tests help decide whether medical management is enough or surgical opinion is required.

💊 Internal Medicines Phase 1 – 30 days (Gland decongestion + Kapha reduction + local inflammation control) 1.Kanchanar Guggulu – 2 tablets twice daily after meals with warm water 2.Varunadi Kashayam – 15 ml + 30 ml warm water, twice daily before food 3.Trikatu Churna – ¼ tsp twice daily before meals with warm water 4.Yashtimadhu (Licorice) churna – ¼ tsp once daily with warm water

Phase 2 – Next 60 days (Size reduction + stability + symptom relief) 1.Continue Kanchanar Guggulu – 2 tablets twice daily 2.Guduchi / Giloy Satva – ½ tsp once daily in the morning 3.Sitopaladi Churna – ¼ tsp with honey, once daily

🌿 External & Supportive Therapies 1.Local oil application (daily, night) Ksheerabala Taila or plain sesame oil Gentle massage over thyroid area (5 minutes only, no pressure) 2.Warm water gargles once daily 3.Avoid constant throat clearing and loud voice use

🥗 Diet & Lifestyle Recommendations ❌ Avoid Cabbage, cauliflower, broccoli (raw or excess) Soy products Cold foods and cold drinks Excess curd, especially at night Packaged and preserved foods

✅ Include Warm, freshly cooked vegetarian meals Cooked vegetables (lauki, carrot, pumpkin) Adequate iodine from normal iodized salt (do not overuse) Ginger, black pepper, turmeric in cooking Warm water throughout the day

🧘🏻‍♀️ Yoga & Pranayama Thyroid-supportive practices Anulom Vilom – 10 minutes daily Bhramari Pranayama – 5 rounds (excellent for voice and throat) Neck movements (gentle) – flexion, extension, rotation Sarvangasana – only if comfortable and under guidance

🕉️ Follow-up & Duration Reduction in cough and throat irritation: 2–3 weeks Gradual reduction or softening of swelling: 2–3 months Long-standing goiter takes time and consistency Repeat ultrasound after 3–6 months to assess size change

⚠️ If swelling increases rapidly, pain develops, or voice worsens significantly, surgical consultation is advised.

✨ With consistent treatment, dietary discipline, and regular follow-up, long-standing euthyroid goiter can be reduced or stabilized, and symptoms like cough and voice change can improve significantly.

With kind regards, Dr. Sumi MS (Ayu)

Hello I get why you’re worried. A swollen thyroid (goiter) for years, plus a funny voice and constant cough, is a lot, even if your blood tests are normal. But dont worry we are here to help you out 😊

YOUR CONCERN

–Left thyroid swelling (goiter): Been with you for about 15 years. –Thyroid hormones (TSH, T3, T4):** All good (meaning it’s a euthyroid goiter). –Symptoms: Voice changes Frequent, mild cough No pain No trouble swallowing food or drinks –Diet: Mostly vegetarian –Energy: Normal

AYURVEDIC UNDERSTANDING In Ayurveda, your condition is pretty close to what they call Gandamala or Galaganda.

–Too much Kapha (one of your body’s energies) causes swelling in one spot. – Problems with Meda (fat tissue) and Mamsa (muscle tissue). – Channels get blocked up over time. – Because it’s been there so long, it’s gotten tough and firm. – It’s pressing on things nearby, which is causing your voice change and cough.

👉 Since your thyroid hormones are fine, this isn’t a hormone problem. It’s a physical growth. Ayurveda can help shrink it or keep it from getting bigger. After 15 years, it might not disappear completely, but we can definitely help

Because this swelling is 15 years old:

–Ayurveda can help make it smaller, softer, and stop it from growing more. –Getting your voice better and coughing less are very realistic goals. –It might not go back to totally normal, but feeling better and controlling the symptoms are totally achievable.

TREATMENT GOAL

–Make the goiter smaller and softer. –Improve your voice. –Lessen throat irritation and cough. –Stop it from growing bigger. –Try to avoid surgery (as long as there are no serious warning signs).

AYURVEDIC PLAN OF TREATMENT

1. INTERNAL MEDICATION

1. Kanchanar Guggulu 2 tablets, twice a day after food. This is a classic medicine for goiters and swollen glands. It helps clear out Kapha and fat buildup and slowly makes the swelling smaller.

2. Varunadi Kashayam 15 ml mixed with 15 ml warm water, twice a day before food. Reduces swollen glands and helps with lymphatic drainage (the body’s way of clearing waste).

3.Sitopaladi Churna Half a teaspoon, twice a day with honey. Calms down throat irritation and helps with cough and voice strain.

EXTERNAL CARE

Kanchanar Bark Powder + warm castor oil** -Mix them to make a paste. -Rub it on the swelling at night. -Lightly cover it with a cotton cloth. -Do this 4-5 times a week. 👉 This helps soften the gland and gets the blood flowing in that area.

DIET AND LIFESTYLE MODIFICATION

❌AVOID (foods that increase Kapha):

* Yogurt at night * Cold drinks * Ice cream * Too much cheese or paneer * Fried and oily stuff * Baked goods, refined flour (maida) * Excess soy products

✅INCLUDE

* Warm, freshly cooked meals * Moong dal (lentils) * Barley, millet * Cooked veggies * Turmeric + black pepper * Warm water all day

👉 Stay away from very cold food/drinks — they make Kapha and throat symptoms worse

Taking care of your voice and throat: –Don’t clear your throat too often. –Do steam inhalation 2-3 times a week. –Have honey with ginger juice in the morning. –Try not to shout or talk loudly for long periods.

Tests you should get (to be safe):

Even though your hormones are normal, make sure you have: –A very recent ultrasound of your thyroid. –A fine needle aspiration cytology (FNAC) test (if you haven’t had one or if the size is growing). – An ENT doctor check your vocal cords because of your voice change.

–Yes, Ayurveda can help you with this. –Goiters where hormones are normal usually respond better than hormone-related ones. –Things that have been there a long time need patience and consistency.

Warm Regards Dr. Snehal Vidhate
