In this case, where there is an elderly patient undergoing dialysis with high creatinine, diabetes, and arthritis, Ayurveda can provide supportive care rather than a direct cure. Dialysis is crucial for managing kidney function, and Ayurvedic treatment should be used as a complementary approach to enhance quality of life and manage symptoms. Always consult with your allopathic physician before incorporating any new treatments.

Supporting Kidney Function: Focus on gentle herbs like Punarnava (Boerhavia diffusa) and Gokshura (Tribulus terrestris), which are traditionally used for supporting renal health. Herbal preparations should be administered with professional guidance due to the complexity of dialysis conditions.

Managing Diabetes: Dietary management is key. Encourage foods that are light and easy on digestion, such as cooked vegetables, whole grains like millet and rice, and legumes in moderation if they don’t interfere with kidney function. Avoid foods high in sugar and refined carbs. Gymnema sylvestre (Gurmar) can be considered to support glycemic control, but only with the physician’s consent.

Arthritis Pain: Low-impact exercises tailored to individual capacity and hot oil massages with sesame or Mahanarayan oil can help alleviate joint pain. Avoid massages during flare-ups or dialysis days to prevent any complications.

Lifestyle Adjustments: Maintain a routine schedule for sleep and meals to balance Vata, which can be disturbed in arthritis and stress. Gentle yoga or pranayama might be beneficial, provided there’s no physical strain.

Abiding Medical Protocol: Adhere strictly to the allopathic care given, as dialysis is essential. Ayurveda here is to enhance comfort and support existing treatment. Sudden stops or changes in prescribed medicines should be avoided.

Addressing Pain: Utilize anti-inflammatory and analgesic Ayurvedic formulations with care. Rasna (Pluchea lanceolata) and Shallaki (Boswellia serrata) are traditionally used for joint health. Again, consult with a healthcare practitioner for these additions.

Ensure close monitoring and coordination with healthcare providers across all disciplines to maintain patient safety and treatment efficacy.

In your situation, Ayurveda may not offer a direct cure for dialysis dependency. However, there are ways to supplement current treatments through herbal and lifestyle strategies tailored to ease symptoms and improve overall well-being. With such a complex condition, integrating supportive measures should be done cautiously alongside ongoing medical advice.

Begin with dietary modifications, prioritizing easily digestible, warm foods for strengthening agni. Rice, moong dal, and steamed vegetables are gentle on the system. Avoid heavy, oily, spicy, and excessively salty foods, as these can stress kidneys and exacerbate pitta imbalances. Keep meals small and frequent to support blood sugar levels in diabetes.

Incorporate herbs like Amla (Indian gooseberry) to support immunity and vitality. It’s rich in vitamin C and can be consumed as powder or in fruit form. Methi (fenugreek) seeds may be beneficial for managing diabetes, but consult with a healthcare provider before use due to possible interactions with medications.

For arthritis, nirgundi oil might provide relief when applied topically to knees. Gentle yoga or massages focusing on relaxation can aid mobility and circulation without straining the body.

Hydration is vital; limit fluid intake as advised by your healthcare team. Stay consistent with medical check-ups and dialysis sessions. Consulting both your Ayurvedic practitioner and a renal specialist ensures a balanced approach.

Remember, while Ayurveda offers supportive care, it should not replace conventional treatment, especially in critical conditions like these where timely care is paramount.
