HELLO,

The epididymis is a thin, coiled tube behind the testicle that stores and carries sperm.
When it becomes inflamed for a long time (more than 3 months) it is called chronic epididymitis

Why does it become chronic? Even when infection settles, residual inflammation, thickening, and poor blood flow remain

Common reasons -Past infection (urinary or sexual) -Long sitting, pressure on scrotum -Stress and anxiety -Pelvic floor muscle tightness -Incomplete healing after antibiotics

Why is there a lump? The lump is usually -Thickened epididymal tissue -Fibrosis (scar-like tissue) -Inflammatory residue

It is not cancer in most chronic cases especially if -Slow growing -Mild pain -Soft to firm (not rock hard)

In Ayurveda this condition is mainly -Vata-Kapha imbalance -Affects Shukravaha srotas (reproductive channels) -Stress aggravates Vata, causing pain, tightness, and chronicity -Kapha causes hardness and swelling

This is why -Pain increases with stress -Symptoms persist for months -Lump reduces slowly, not immediately

Hemorrhoids – Why You Have Them Together Hemorrhoids indicate -Apana Vata disturbance (downward energy) -Pelvic congestion -Constipation or straining - Both epididymitis and piles share the same pelvic circulation problem so treating one helps the other.

Can This Affect Erection & Pelvic Floor? Yes indirectly

Reasons -Chronic pain =nervous system tension -Pelvic inflammation =pelvic floor muscle spasm -Stress = reduced blood flow & confidence -Fear of pain =weak erection response This is functional not permanent.

TREATMENT GOALS • Reduce chronic inflammation • Improve blood & lymph circulation • Soften fibrotic lump • Calm Vata (pain, stress) • Correct pelvic floor dysfunction • Improve bowel movement • Restore sexual confidence

INTERNAL MEDICATIONS

1) Kanchanar Guggulu= 2 tablets twice daily after food for 8–12 weeks =Breaks down glandular swelling, Softens hard lumps, Reduces fibrosis

2) Punarnava Guggulu= 2 tablets twice daily after meals for 6-8 weeks
=Reduces chronic inflammation, Improves local circulation, Removes fluid congestion

3) Chandraprabha Vati= 2 tablets twice daily after meals for 8 weeks
=Balances pelvic organs, Supports urinary & reproductive systems, Relieves dull pain and heaviness

4. Dashamoola Kashaya= 25ml once daily after dinner for 6 weeks =Powerful anti-inflammatory, Best medicine for chronic Vata pain, Improves nerve function

5. Ashwagandha Churna= 1 tsp with warm milk at night =Controls stress trigger, Improves nerve healing, Supports sexual health

EXTERNAL THERAPY

1. Local Oil Application Oil: Bala Taila
Method: -Apply gently around scrotum (not squeezing) -Massage for 10 minutes -Follow with warm compress Frequency: Daily
Duration: Minimum 6 weeks =Relaxes tissues,Improves blood supply, Softens lump gradually

2. Sitz Bath Method: -Warm water -Add Dashamoola decoction or Epsom salt -Sit for 10–15 minutes Frequency: 3–4 times/week =Relieves pelvic congestion, Helps both epididymitis & piles

HEMORRHOIDS – DETAILED AYURVEDIC CARE Internal Medicines

-Triphala churna – 3–5 g at bedtime

-Abhayarishta – 15 ml at night with warm water

-Arshoghni vati – if pain/bleeding

Local -Jatyadi taila application after bowel movement

Goal -Soft stool -No straining -Reduced pelvic pressure

DIET

Avoid -Spicy, fried, fermented food -Alcohol, smoking -Excess tea/coffee -Processed food -Cold drinks

Include -Warm freshly cooked food -Ghee (1–2 tsp daily) -Pumpkin seeds -Coconut water -Turmeric + black pepper -Papaya, figs, soaked raisins

Yoga Asanas -Malasana -Baddha Konasana -Supta Baddha Konasana -Happy Baby pose -Gentle pelvic rocking Avoid forceful core workouts or cycling during pain

Pranayama -Anulom Vilom – 10 minutes -Bhramari – 5 minutes -Deep belly breathing Avoid aggressive Kegels initially

HOME REMEDIES -Warm water every morning -Turmeric milk at night -Castor oil pack on lower abdomen (weekly) -Avoid holding urine or stool

INVESTIGATIONS ADVISED (ONCE) To rule out serious causes • Scrotal ultrasound with Doppler • Urine routine & culture • ESR / CRP (inflammation markers) • Blood sugar (if healing is slow)

9. EXPECTED IMPROVEMENT -Pain= 3-4 weeks -stress response = 2-3 weeks -lump softening= 6-12 weeks -bowel improvement- 1-2 weeks -sexual confidence= 4-8 weeks

-Chronic epididymitis is not dangerous -It requires patience and consistency -Surgery is rarely needed -Stress control is as important as medicine -Healing is gradual but reliable

DO FOLLOW

HOPE THIS MIGHT BE HELPFUL

THANK YOU

DR. MAITRI ACHARYA

Hello Thanks for sharing your concerns so openly. I get how tough it is to deal with chronic epididymitis and a scrotal lump, especially when you’re also managing hemorrhoids and worrying about your sexual health.

YOUR CONCERN

- Chronic epididymitis with a scrotal lump that’s stuck around for more than six months - Hemorrhoids (piles) - Stress as a trigger - You keep a fairly active lifestyle and eat well

YOUR MAIN WORRIES - Avoiding surgery - Getting rid of the scrotal lump - Finding the best herbal remedies for hemorrhoids - Understanding how all this affects erections and pelvic health

First off, I hear you. When you’re dealing with inflammation in sensitive spots like the epididymis and rectum, it’s easy to start wondering if this is permanent, or if your sex life is at risk.

Here’s the thing — most chronic cases like yours can be managed without surgery. Ayurveda isn’t just about calming symptoms; it’s about tackling inflammation, improving circulation, and making tissues stronger.

AYURVEDIC PERSPECTIVE

Epididymitis lines up with Vata–Pitta imbalance in the Shukravaha Srotas. That leads to inflammation, sometimes fibrosis or a lingering lump, and often pain or heaviness. Hemorrhoids (Arsha), on the other hand, come from Apana Vata imbalance — usually from constipation, straining, or weak blood flow in the rectal area. Both issues, at their core, have disturbed Apana Vata.

AYURVEDIC PLAN OF TREATMENT

INTERNAL MEDICATION

- Kanchanara Guggulu: 1 tablet twice daily after food Shrinks lumps, breaks down fibrotic tissue, and helps the scrotal lump go down over time.

- Punarnava Mandura: 1 tablet twice daily after food Boosts micro-circulation, eases local swelling, and keeps fluids from pooling.

- Gokshuradi Guggulu: 1 tablet twice daily after food Strengthens your urinary and reproductive systems, balances Apana Vata, and helps when stress makes things worse.

- Chandraprabha Vati: 1 tablet twice daily after food Fights inflammation, supports pelvic organs, and eases pain and heaviness.

EXTERNAL CARE - Gentle oil massage (Abhyanga) with Bala Taila or Mahanarayana Taila around the groin and lower abdomen (be gentle, no rough rubbing).

- Daily warm sitz baths for 10-15 minutes relax the pelvic floor, boost blood flow, and help with both epididymitis and hemorrhoids.

FOR HEMMORRHOIDS

- Abhayarishta: 15mll at bed time
Keeps your bowels moving regularly and reduces straining (that’s the big one).

–Triphala Churna ½ tsp with warm water : Mild laxative, improves tone, and helps keep piles from getting worse.

- Arshoghni Vati 1 tablet twice daily Shrinks the piles, eases pain and inflammation.

- Jatyadi Taila: For external use if you’ve got burning or pain.

NOW ABOUT ERECTION AND PELVIC FLOOR

- Epididymitis doesn’t directly harm erectile function. But chronic pain, stress, and fear can lead to temporary weakness or performance anxiety.

- Hemorrhoids don’t damage the nerves in the penis, but chronic constipation and straining can weaken pelvic floor muscles and shake your confidence in the bedroom.

- Once Apana Vata is back in balance, sexual function usually gets better on its own.

LIFESTYLE CHANGES

- Don’t sit for long stretches. - Never hold in urine or stool. - Go for gentle walks; skip heavy cycling. - Find ways to manage stress — that’s especially important for you.

Surgery? Only if the lump keeps growing, there’s a severe infection, an abscess, fever, or things don’t improve even after good therapy. Most stable, chronic cases like yours don’t need surgery.

you’re not facing a dead end. With the right Ayurvedic medicines, bowel care, and Vata balancing, both epididymitis and hemorrhoids can be managed and improved without surgery. Erectile function is usually just fine.

Warm Regards Dr Snehal Vidhate
