I’m Suffering with Sciatica from 2010 first time pain was going and coming off and ON till late 2014. Then no pain till march 2018 lasted till December 2018. It came back again very badly from 07/04/26 with a loss of sensation in left leg under the knee front side of leg, it feels like anaesthetic leg just front part between knee and toe. Back pain is still there. Having problems with L3-L4 and L4-L5 and L5-S1. MRI—- report At L3/4, there is a posterior annular fissure with a posterior central disc protrusion. There is moderate bilateral facet joint arthropathy resulting in moderate spinal canal stenosis. There is moderate bunching of the cauda equina nerve roots with effacement of CS signal. There is also a superimposed left posterior disc sequestration measuring approximately 12 x 7 x 17mm posterior to the L4 vertebral body. This is contributing to impingement of the descending L4 nerve root at the inferior aspect of the lateral recess. No exiting nerve root impingement. At L4/5, there is circumferential disc bulge with a posterior central disc protrusion with inferior disc extrusion. This is resulting in moderate spinal canal narrowing with moderate bunching of the cauda equina nerve roots. Narrowing of the lateral recesses, impinging on the descending L5 nerve roots, right greater than left. There is mild bilateral neural exit foraminal narrowing. Moderate facet joint arthropathy. At L5/S1, there is a posterior central disc protrusion with bilateral facet joint arthropathy. Patent spinal canal and lateral recesses. There is moderate bilateral neural exit foraminal stenosis, impinging on the exiting L5 nerve roots. Conclusion- There is moderate spondylosis at L3/4 with a circumferential disc bulge with a superimposed posterior central disc protrusion. There is a sequested disc fragment seen at the left lateral recess, impinging on the descending left L4 nerve root. Moderate to severe spinal canal stenosis is seen at this level with bunching of the cauda equina nerve roots and severe effacement of CSF signal. There is also moderate to severe narrowing of the right lateral recess, impinging on the descending right L4 nerve root. At L5/S1, there is a large posterior central disc protrusion with inferior disc extrusion. There is bunching of the cauda equina nerve roots and moderate spinal canal narrowing. Bilateral facet joint arthropathy is seen. There is bilateral narrowing of the lateral recesses, potentially irritating the descending L5 nerve roots, right greater than left. Neurosurgery opinion is recommended. The patient has been asked to return to the referring medical center for review.

Dealing with chronic pain since 2010 is exhausting, and the sudden onset of numbness (the “anaesthetic” feeling) is a significant clinical shift. ​ ​Your MRI shows sequestration (a fragment has broken off), cauda equina nerve root bunching, and moderate to severe spinal canal stenosis. These are serious findings. ​ If you experience any loss of bladder or bowel control, or numbness in the “saddle area” (groin/inner thighs), please go to the Emergency Room immediately. This can indicate Cauda Equina Syndrome, which requires urgent surgery to prevent permanent damage. ​ ​In Ayurveda, this condition is known as Gridhrasi. It is primarily a Vata disorder (specifically Vyana Vayu and Apana Vayu), often complicated by Kapha (stiffness/blockage). ​The disc sequestration and nerve compression are seen as a severe “Vata blockage” where the flow of energy and blood is obstructed. The loss of sensation indicates that the Vata has caused significant depletion (Dhatu Kshaya) in the nerve tissues. ​ ​1. Sahacharadi Kashayam Liquid : This is the primary formula for Vata issues affecting the lower limbs. ​Dose: 15ml liquid mixed with 45ml lukewarm water (or 2 tablets) twice daily, 30 minutes before food. ​2. Ksheerabala (101) Capsules A fortified version of Ksheerabala oil, specifically processed 101 times to strengthen nerves and reduce “emptiness” or numbness. ​Dose: 1 capsule twice daily after food with warm water. ​3. Dhanwantharam Gulika Helps in managing the downward flow of Apana Vayu and relieving the pressure sensation in the spine. ​Dose: 1 pill twice daily, crushed and taken with the Sahacharadi Kashayam. ​4. hello, i hope your consultant has adequately informed you regarding your current condition. some structural disturbance has occured in your spinal canal and these needs to be corrected as soon as possible to prevent further damage to the nerve roots. you need to meet with a neurosurgeon and listen to his advise on surgery. And you have to prepare your mind that your symptoms will not bounce back soon after the surgery because your nerves has sustained considerable damage throughout this period and nerve healing and recovery is a slow process. this is the stage ayurveda can act on. there is a physical fragment that is compressing your spinal cord and unless it is corrected no amount of external manuvering is going to be much help. but you can apply lepas externally for temporary relief. once your structural issues are resolved you can stress more on ayurvedic treatments to correct the functional aspects and improve pain and nerve healing. also stay away from chiropractise traction etc as these will not suit your condition. also please start the medication mentioned to sooth your nerves temporarily and help with pain. these are herbal formulations that can strengthen your nerves. follow a light diet and mild exercises.
