Since your wife has severe pain, worse in the morning and on movement, it suggests active Vata imbalance with early inflammatory changes. Ayurvedic Treatment Approach 1. Internal Medicines Yograj Guggulu – reduces joint stiffness, pain, and inflammation Simhanad Guggulu – helps in early arthritis with Ama detox Ashwagandha Churna / Capsules – strengthens muscles and joints Dashmool Kwath – balances Vata and reduces body pain Shallaki (Boswellia) – natural anti-inflammatory support 2. Internal Medications (Aushadhi Chikitsa) ​To tackle both the localized knee pain and the systemic body aches, a combination of Vedanahara (pain-relieving), Amapachana (toxin-digesting), and Vatashamana formulations is required: ​For Toxin Digestion & Deep Inflammation (Ama & Vata Management) ••​Simhanada Guggulu or Yogaraja Guggulu: 2 tablets, twice a day, after food. ​Clinical note: If she has a coated tongue, loss of appetite, or signs of Ama, Simhanada Guggulu is highly preferred as it contains Gandhaka and Haritaki to help clear bowel toxins and reduce acute joint swelling. ••​Rasnasaptak Kwath or Maharasnadi Kadha: 15–20 ml with an equal quantity of lukewarm water, twice a day before or after meals. ​Why: Rasna is the premier Vata-hara herb for pain originating from the lower back down to the knees and feet. ​For Acute Pain & Radiating Body Aches ••​Vatagajankusha Ras or Amavatari Ras: 1 tablet (125 mg to 250 mg) twice a day. ​Anupana (Vehicle): Best administered with warm water or a small dash of honey/ginger juice. These potent Rasashastra formulations act rapidly on acute pain pathways. ​2. External Applications (Bahya Chikitsa) ​⚠️ Crucial Clinical Guardrail: Check for local inflammation first. •​If the knee joints are hot to the touch, visibly swollen, or throbbing, do not perform heavy oil massage (Snehana). Instead, apply Lepa (dry herbal pastes) or do Valuka Sweda (sand fomentation). •​If there is no heat or active swelling, and it is a dry, cracking pain, proceed with therapeutic oils. •​For Dry/Vata Pain (No Heat/Swelling): Apply Mahanarayan Taila mixed with Kottamchukkadi Taila gently over the knees and body. Follow with mild steam or a warm water bath. •​For Active Inflammatory/Ama Pain (Hot/Swollen): Apply Kottamchukkadi Churna or Grihadhoomadi Churna mixed with warm water or tamarind leaf juice as a Lepa (paste) over the knee for 45 minutes, then wash it off with warm water. ​3. Dietary & Lifestyle Protocols (Ahara & Vihara) ​Medication will only work if the Agni (digestive fire) is preserved. In acute phases, strict dietary compliance prevents further Vata vitiation: ••​What to favor: Warm, freshly cooked, easily digestible foods like Moong Dal Khichdi, vegetable soups seasoned with cumin, ginger, and turmeric, and strictly warm water for drinking. ••​What to strictly avoid: Curd, sour foods, fermented foods (Idli, Dosa), bakery items, deep-fried dishes, and Vata-increasing vegetables like potatoes, cauliflower, and ladyfinger (Bhindi). ​Rest: Complete rest for the affected joint during the acute flare-up is essential to prevent micro-trauma to the synovial tissue.
