Hello Alikoya,
I can understand your concern regarding chronic back pain with disc bulge and knee pain with reduced joint space. Your case indicates Vata–Kapha imbalance, degenerative changes in spine and knee (Asthi–Majja dhatu kshaya), and weakness of supporting muscles). The approach will focus on Vata shamana, joint & bone nourishment, strengthening muscles, and maintaining mobility.
🔎 Recommended Investigations 1. Repeat X-ray / MRI of spine and knee – to monitor disc and joint changes. 2. Vitamin D3 & Calcium – bone health. 3. Serum uric acid – rule out gout if joint pain worsens. 4. Basic blood tests – ESR, CRP for inflammation.
💊 Internal Ayurvedic Remedies For Back & Knee Pain (Vata Kapha)
1. Mahayogaraj Guggulu – 2 tablets twice daily after food 2. Ashwagandha Churna – 1 tsp with warm milk at night 3. Rasnasaptaka Kwatha – 15 ml with 30ml of warm water twice daily before food
🍀 External Therapies 1. Abhyanga (oil massage) – with Mahanarayan taila, daily or at least 4–5 times/week. 2. Kizhi (herbal bolus massage) or Patra Pinda Sweda – 2–3 times/week (reduces stiffness, nourishes joints). 3. Medicated warm fomentation – 15–20 min on knee/back before exercise.
🥗 Diet & Lifestyle ✅ Include: - Warm, cooked meals – khichdi, dal, seasonal vegetables. - Milk with turmeric or ghee for bone nourishment. - Nuts & seeds – almonds, sesame, walnuts for joint strength.
❌ Avoid: - Cold, raw, and refrigerated foods (increase Vata). - Excess coffee, tea, fried foods. - Long sitting on hard surfaces; heavy lifting.
🧘🏻♂️ Yoga & Gentle Exercises 1. Bhujangasana, Setu Bandhasana, Ardha Matsyendrasana – improve back flexibility. 2. Shalabhasana, Supta Padangusthasana – strengthen back & hamstrings. 3. Chair-assisted squats, leg raises, walking – strengthens knee muscles. 4. Avoid jerky movements or deep bending; maintain correct posture.
📌 Follow up
Pain often recurs if medicines are stopped suddenly; gradual taper with lifestyle, exercise, and diet support is essential. Long-term management: Ayurvedic medicines + external therapies + exercises can reduce pain and improve mobility, but degenerative changes cannot be completely reversed. Regular follow-up every 3–6 months recommended.
✨ With consistent Ayurvedic treatment, strengthening, and posture correction, your back and knee pain can be significantly reduced, improving daily function and quality of life.
With kind regards, Dr. Sumi
Hello Alikoya I can understand your concern about disc bulge and knee pain but dont worry we are here to help you out😊
–From your history, your back pain started after a fall and X-ray shows disc bulge with gap (lumbar disc problem), –now you also have knee pain due to cartilage/ fluid reduction (early osteoarthritis).
Both are chronic issues where pain increases when medicine is stopped, which shows that inflammation and degeneration are the root causes.
✅Why Pain & Inflammation Persist??
➡️Disc bulge causes nerve compression → back pain, stiffness, radiating pain. ➡️Loss of knee joint fluid & cartilage thinning → friction, swelling, pain. ➡️Stopping medicine allows Vata dosha to aggravate again, leading to pain flare-up. ➡️Ageing also increases Vata, which naturally causes degeneration.
AYURVEDIC PLAN. OF TREATMENT- This is progressive degenerative condition as joints are weight bearing structure of body it will undergo daily wear and tear which will further progress the degeneration
Focus of TREATMENT- ✅Reduce the pain and inflammation ✅ Prevent Further Degeneration ✅ Providing strength to the joints
For this you must follow INTERNAL MEDICATION ALONG WITH 8 DAYS PANCHAKARMA THERAPIES
✅INTERNAL MEDICATION ( 45 DAYS) 1 Yograj Guggulu 1-0-1 after food – for chronic joint & back pain.
2 Rasnadi Kashayam 15ml-0-15ml +45 ml water – reduces inflammation & stiffness.
3 Shallaki (Boswellia) Capsules 1-0-1 after food – support cartilage & joint health.
4 GT 2 cap at bed time
✅PANCHAKARMA THERAPY - for 8 days ( Visit nearest Ayurvedic center for the treatment )
1 Sarvanga Abhynga with Mahanarayana taila followed by Dashmool parishekha ( body massage with medicated oil followed by medicated decoction bath )
2 Meru basti - whole spine will be covered with warm. Medicated oil
3 Yoga basti ( helps to pacify vaata and prevent further degeneration)
4 Janu basti- warm oil retention on both knees
✅HOME. REMEDIES AND DAILY CARE
1. Hot fomentation (Nadi Sweda or warm compress) – Apply warm towel/ heating pad over back and knees daily for 10–15 min to reduce stiffness and pain.
2. Castor Oil (Eranda Taila) – 1 tsp warm castor oil at night once or twice a week helps reduce Vata, constipation and inflammation.
3. Turmeric + Milk – ½ tsp turmeric in warm milk reduces joint inflammation naturally.
4. Fenugreek (Methi) Seeds – Soak overnight, chew in morning – good for joint lubrication.
5. Gentle Yoga/Stretching – Bhujangasana, Pawanmuktasana, and light knee strengthening exercises under guidance maintain flexibility.
✅ DIET AND LIFESTYLE CHANGES
👉Take warm, light, easily digestible diet – moong dal, soups, vegetables, ghee in small quantity.
❌Avoid cold, stale, very dry food (like bakery items, refrigerated food).
👉Maintain ideal weight to reduce stress on knees.
👉Walk slowly on even ground, avoid sitting on floor or climbing stairs frequently.
👉Sleep on a firm mattress (not too soft), avoid sudden jerky movements.
Your back and knee pain are due to disc bulge (spinal degeneration) and loss of knee joint lubrication (Sandhigata Vata).
With regular care and therapies like Janu Basti & Kati Basti, pain can be controlled and degeneration slowed down.
Wish you a good health😊
Warm Regards Dr Snehal Vidhate

HELLO ALIKOYA,
1) BACK PAIN- DISC BULGE AND GAP -In the spine, there are discs that act like cushions between the bones -With age or injury (like your fall 5 years ago), the discs can budge or lose height-> leading to pain, stiffness, and sometimes nerve compression -Ayurveda calls this a vata disorder- dryness, degeneration, loss of cushioning
2) KNEE PAIN- Loss of joint fluid -normally, joints have synovial fluid (like natural oil ) that makes movements smooth -With aging, wear and tear, or imbalance, this fluid reduces-> bones rub directly, causing pain, creaking, swelling, and stiffness -Ayurveda calls this Sandhigata vata (vata entering the joints, drying them )
So, both conditions come from excess vata-> dryness, degeneration, and pain
TREATMENT GOALS -reduce pain and inflammation -improve lubrication in joints and spine -strengthen muscles and bones to prevent further degeneration -correct digestion and metabolism-> because poor digestion worsens vata -slow down progression of arthritis and disc degeneration
INTERNAL MEDICATIONS
1) MAHAYOGARAJ GUGGULU= 1 tab thrice daily after mels =reudces pain, inflammation, remove stiffness, supports joints

DURATION= usually 3 months reviewed every 4-6 weeks
OIL MASSAGE WITH MAHAARAYAN TAILA over affected area followed by warm water bath
DIET -warm, soft, slightly oily foods- khichdi, rice with ghee, moong dal soup, milk, stews -spices= ginger, cumin, turmeric, ajwain-> improve digestion, reduce inflammation -Nuts and seeds in moderation= soaked almonds, sesame seeds -Herbal teas with ginger, cumin
AVOID -dry, cold, raw, processed foods -excess tea, coffee, refrigerated food -Long fasting, irregular meals
LIFESTYLE -Avoid heavy lifting, sudden bending, jerky movements -gentle daily massage before warm shower -regular sleep avoid late nights -short walks, not prolonged standing or sitting
YOGA (gentle, avoid strain) -makarasana= relaxes lower back -Setubandhasana= strengthens back and knees -Bhujangasana= improves flexibility of spine -Tadasana= posture correction
PRANAYAM -Anulom vilom= balances vata -Bhramari= relaxes nervous system -Deep diaphragmatic breathing= reduces stress which worsens pain
HOME REMEDES -turmeric milk daily at night -fenugreek seed powder 1/2 tsp in warm water for joint strength -castor oil with warm milk 1 tsp at bedtime, once or twice a week for vata balance -warm sesame oil massage on knees and back followed by warm compress -ajwain hot fomentation on knee/back for pain relief
INVESTIGATIONS ADVISED -X-ray of knee and spine -MRI of lumbar spine
-your condition is chronic and degenerative so goal is not “complete cure” but pain reduction slowing further damage, and improving daily activity -Ayurveda has excellent long term management for such vata disorders -The best results usually come from internal medications, external therapies, regular lifestyle changes, yoga and diet -Patience is key- treatment of often gradual but sustainable
THANK YOU

Given the chronic nature of your back and knee pain, it’s crucial to approach your condition with a comprehensive Ayurvedic strategy. Your case involves vata imbalance, particularly with the kshaya (depletion) in the joints and spine, which aligns with disc bulge and fluid loss.
First, addressing Agni is important. Incorporate easily digestible foods, staying clear from heavy, oily, or cold substances which can aggravate vata. Start meals with ginger tea or add some black pepper to warm up digestion.
For diet, favor warm, cooked meals—soups and stews with ingredients like rice, quinoa, or barley. Ghee is beneficial, so consuming it in small amounts can help lubricate joints. Turmeric and ashwagandha, known for anti-inflammatory and strengthening properties, should be added to your diet.
Apply warm sesame oil to your back and knees before bathing, which helps to nourish and provide gentle heat. This can be followed by a warm bath infused with epsom salt for muscle relaxation.
Panchakarma therapies, if accessible, such as Basti (therapeutic enemas) have shown benefits for chronic vata disorders. Regularity, however, is key, so ongoing treatment under a practitioner’s supervision is advised.
Simple yoga poses, emphasizing on stretching and flexibility without straining, can help. Cat-Cow stretch and gentle forward bends can maintain spine mobility. Avoid postures that pressure the lower back or knees.
Consider shirodhara or abhyanga to stabilize mental stress which can exacerbate pain perceptions. Such treatments reinforce balance and vata stability.
Finally, if pain persists, consult your practitioner for possible integrative treatments. Do not discontinue medications abruptly and discuss thoroughly with healthcare providers for any adjustments in your regimen. Remember, chronic conditions require persistent care and patience.
Take 1) maharasnayograj gugulu 15ml+45ml lukewarm water subha sham khane ke baad 2) ointment shalkii for local aplication 3) Tab. Tryodhshang gugulu 2 goli subha sham khane ke baad

Addressing back and knee pain requires a strategic approach—balancing immediate relief and long-term healing. The disc bulge and gap in your spine point towards a vata imbalance, often aggravated by injuries. For your knee, the absence of synovial fluid suggests kapha depletion, affecting proper joint lubrication.
For back pain, consider enhancing your vata balance by incorporating gentle yoga and stretching exercises focused on the spine. Practice specific asanas like Bhujangasana (Cobra Pose) and Marjariasana (Cat-Cow Pose) daily. This can help improve flexibility and reduce pain. Additionally, apply warm sesame oil or Mahanarayan oil to your back in circular motions. Consistent abhyanga (oil massage) helps nourish the tissues and reduces vata dosha.
For knee pain, increase kapha elements by consuming more moisturizing and joint-supportive foods. Include ghee, warm milk, and soups in your diet. You might find it beneficial to take supplements like Shallaki (Boswellia) and Guggulu, known for their anti-inflammatory properties. Apply a warm paste made of ginger and turmeric on the knees to reduce inflammation.
Check your digestion since proper agni ensures nutrient absorption for tissue repair. Consume turmeric, black pepper, and a pinch of asafoetida to boost your digestive fire. Avoid cold and stale foods, which can disrupt internal balance.
Be consistent with these practices and regularly consult with your Ayurvedic physician to tailor ongoing treatment. If pain persists or worsens, discuss the possibility of additional interventions, like panchakarma therapy or external therapies, that can support significant healing.
