••Primary Priorities: Control Bleeding & Soften Stool
In an elderly patient, we avoid aggressive purgatives. Instead, we use gentle Anulomana (medicines that guide Vata downward smoothly) and Stambhana (hemostatic herbs to stop bleeding). The following classical formulations are widely available at standard Ayurvedic pharmacies (such as Baidyanath, Dabur, Kottakkal, or Patanjali):
To Stop the Bleeding:
••Nagkesar Churna / Bol Parpati:
•What it is: Nagkesar (Mesua ferrea) is an exceptional herb for cooling the body and stopping active capillary bleeding.
•Dosage: Typically, 1 to 2 grams of Nagkesar churna mixed with a teaspoon of fresh butter or a little water, taken twice a day after meals.
••Arshoghni Vati:
•What it is: A classical tablet formulated specifically to check bleeding and reduce the size of pile masses.
•Dosage: 1 to 2 tablets twice a day with water after meals.
To Soften the Stool & Prevent Straining:
Abhayarishta:
•What it is: A mild, fermented liquid preparation where the main ingredient is Haritaki (Terminalia chebula). It is the premier Ayurvedic tonic for piles because it softens stool naturally without causing cramping or diarrhea.
•Dosage: 15 to 20 ml mixed with an equal quantity of lukewarm water, taken twice a day after meals.
••Triphala Churna or Gandharvahastadi Castor Oil:
•What it is: If she prefers a powder, a gentle dose of Triphala at night helps clear the bowels. Alternatively, half a teaspoon of Gandharvahastadi Eranda Taila (castor oil processed with herbs) in a cup of warm milk at bedtime provides excellent lubrication to dry intestines.
2. Topical Comfort (Local Management)
To soothe the local area, promote healing, and ease bowel passage, you can look for:
••Jatyadi Taila (or Jatyadi Ointment):
This is a highly effective, soothing herbal oil containing neem, jasmine, and turmeric. Applying a few drops locally before and after passing stool lubricates the passage, reduces burning, and heals local tears (fissures).
3. Crucial Dietary Changes
Medicines alone cannot override chronic constipation. Her diet needs to be highly lubricating (Snigdha) and easy to digest:
••Cow’s Ghee: Introduce 1 to 2 teaspoons of pure cow’s ghee into her warm meals daily. Ghee directly counteracts the dryness in the colon that causes hard stools.
••Soaked Raisins & Figs: Soak 10 to 12 black raisins and 2 dried figs in water overnight. Have her eat them and drink the water first thing in the morning.
••Hydration: Ensure she drinks warm or lukewarm water throughout the day. Cold water slows down digestive fire (Agni).
••Avoid: Strictly eliminate dry foods (like biscuits, toast, rusks), spicy dishes, and heavy lentils until her symptoms completely resolve.
