Your ultrasound findings are consistent with submandibular sialolithiasis (salivary duct stones) with obstruction of the duct. Two stones measuring about 5 mm and 6 mm, along with a dilated duct and swollen gland, suggest that the saliva flow is significantly blocked. ----What this means -Pain and swelling that worsen during or after eating are typical symptoms. -Reduced saliva production occurs because the stones obstruct the duct. -Since symptoms have been present for 6–9 months, spontaneous passage of stones is less likely, especially with stones of this size. --------Rx 1) Kanchanar Guggulu – 2 tablets twice daily after food with warm water. 2) Punarnavadi Kashayam – 15 ml + 45 ml warm water twice daily, 30 minutes before food. 3) Triphala Guggulu – 2 tablets twice daily after food. 4) Gandusha (Oil Holding) 5) Irimedadi Taila or Tila Taila (sesame oil) 10–15 ml -Hold in mouth for 5–10 minutes every morning. -----Local Treatment Warm saline gargles 3–4 times daily. Warm fomentation over the swollen submandibular region for 10 minutes, 2–3 times daily. Gentle massage of the gland from behind forward toward the floor of the mouth after warm fomentation. -----Pathya (Recommended Diet) Warm water throughout the day. Lemon water (if tolerated), citrus fruits. Light, easily digestible meals. Vegetable soups and thin mung dal preparations. -----Apathya (Avoid) Cold drinks, ice cream. Fried, heavy, oily foods. Tobacco and alcohol. Prolonged dehydration. -----Follow-up: 4 weeks.

••Here is a clinical perspective and a structured Ayurvedic approach tailored for this specific pediatric case: 1. Clinical Assessment & Principles of Management ••The Challenge: The USG reveals two distinct stones (5 mm and 6 mm) in the proximal and distal left submandibular duct. Given the tiny diameter of an infant’s salivary duct, 5–6 mm stones are relatively large and are actively causing mechanical obstruction, resulting in the noted ductal dilation (Srotas Vikriti) and glandular inflammation (Shotha). Treatment Goals: Reduce the acute inflammation and swelling (Shothahara). Gently lubricate the duct to encourage spontaneous passage (Snehana and Anulomana). Prevent secondary bacterial infection (Sialadenitis) using safe, natural antimicrobials. 2. Line of Treatment (Chikitsa Sutra) A. External Protocols (Sthanika Chikitsa) Because internal administration is highly restricted by age, localized external treatments are your safest and most effective tools. ••Mild Snehana & Gentle Marma Massage: Apply a few drops of lukewarm Ksheerabala Taila or Eranda Taila (Castor oil) externally over the left submandibular region. Very gently massage in a downward and forward direction (from the angle of the jaw toward the front of the mouth/floor of the tongue). This helps lubricate the duct and mechanically coaxes the distal stone toward the opening. ••Mild Swedana (Local Fomentation): Follow the massage with a very gentle, lukewarm Nadi Sweda or a warm compress using a soft cloth dipped in a decoction of Dashamoola or Dashamoola + Neem (to combat inflammation and potential infection). Ensure the temperature is perfectly safe for infant skin. B. Safe Internal Support (Aushadha) Since the infant is likely on breastmilk and/or early solids, any internal intervention must be highly diluted and given in minute pediatric dosages, mixed with Madhu (honey) or Ghritha (ghee) as a vehicle (Anupana). ••Kanchanara Guggulu: Known for its excellent Granthi-Shothahara (anti-tumor/anti-swelling) properties. ••Pediatric Dosage: Quarter of a tablet (1/4 tab), thoroughly crushed into a fine powder, mixed with honey, twice a day after feeds. Avipattikar Churna or mild Punarnava preparations: To help balance Vata-Kapha and clear the channels. A tiny pinch of Punarnava Churna can be given with honey to reduce the glandular edema. ••Mother’s Diet (If Breastfeeding): If the infant is still breastfeeding, the mother should strictly follow a Kapha-Vata pacifying diet. She should consume warm, easily digestible foods, ginger water, and avoid heavy, sticky dairy (Abhishyandi foods) or cold items, as her diet directly impacts the quality of the breastmilk (Stanya).
