At 86 with chronic respiratory failure + rib pain on movement, this is mainly Vata aggravation (dhatu kshaya, mamsa-asthi kshaya) with muscle weakness. Management should be gentle, nourishing, and pain-relieving 1. Internal medicines (low dose, safe) Dashmoolarishta – 10 ml + equal warm water, twice daily after food Ashwagandha churna – 2 g with warm milk at night Lakshadi Guggulu – 1 tablet twice daily after food (for bone/muscle support) 2. Pain & muscle weakness Yograj Guggulu – 1 tablet twice daily after food 3. External therapy (very important) Mahanarayan taila or Ksheerbala taila Gentle massage on ribs/back once daily, followed by mild warm fomentation (no strong heat) Supportive care Warm, soft diet with ghee (easy to digest) Avoid cold exposure, AC air directly Proper back support while sitting and turning
