From past 3 months i am suffering from some serious symptoms like Itching in urethra Pain in testes and pelvic area Penile numbness Frequent urination Frequent urination is not continuously I have taken two types of treatment First one includes Tab Alfuzosin 10 mg Second is Amitriptylline 10 mg plus Pregabalin 75 mg Both thses medication gives me relief but this relief is only when i continuously take medication Can anyone tell me about this condition so that i can take treatment accordingly

Hi Randeep, Your condition is a chronic pelvic pain syndrome. Urethral itching are also related to the post infectious features.

Along with some medication and lifestyle modification you get relieve from your symptoms.

1. Punarnavadi kashaya 15 ml+ 45 ml lukewarm water twice daily before food. ( To reduce any inflammatory conditions)

2. Gokshuradi guggulu 1-0-1 after food. ( Supports prostate and urinary discomfort)

3. Chandraprabha vati 1-1-1 after food. ( To relieve urinary tract infection and nerve irritation)

4. Triphala water sitz bath for 15 minutes in alternative days.

🛑Avoid prolonged sitting. Avoid spicy, acidic foods, caffeine and alcohol.

Do some streching exercises and yoga postures ( Setu bandhasana and suptha badha konasana are some yoga postures which strengthen the pelvic muscles )

Hi Randeep, pain in testes is caused by several reasons which includes Varicocele, Hydrocele, Epidydimitis , Some testicular injuries, etc , and penile numbness is caused by some nerve damage /or in severe infections like STI ,Conditions like diabetes neuropathy, etc, since your diagnostic reports are normal there is nothing to worry about those . So please start the medication . May be a mild infection can cause these irritations

Yes i have done blood,urine and semen culture. All are in normal range I have done Ultrasound of abdomen and scrotum. This was also normal

I don’t think you have nerve damage, some inflammation/edema/Swelling around the nerves is causing this numbness. So we have to treat this inflammation, for that this set of medication is enough, follow this set of medicines for 2 weeks. Then you can have * Balaguluchyadi kwatham tab (kottakkal) 2-0-2 after food for 2weeks - 1month (For nerve health )

Yes i have done Initial analysis was Cyst in right epididyms Seminal vesculitis Further tested for semen culture and found E.Coli Doctor said that cyst is not harmfull And for <link removed>li doctor gave me 21days antibitotic treatment and again tetes the semen and found that the <link removed>li is not present in semen now

You got some relief from Alfuzosin (for urinary muscle relaxation) and Amitriptyline + Pregabalin (for nerve desensitization) this confirms the issue is linked to pelvic nerve irritation, congestion, and vata imbalance, not just infection.

In Ayurveda, this is mainly a Vata disorder, with some Pitta involvement. In the begining when numbness started i went to a urologist and told my situation. Doctor said that it is chronic prostatitis and prescribed medicine(Alfuzosin and Amitriptylline) But there was no relief Again he added some antibiotic and done prostatitis massage for 1 week on alternate days. Then i felt some relief in pain only . Again he continued same medicine. During this treatment i felt relief in urethra itching when i took Amitriptylline and now the pain was also reduced but numbness was as before . Now i went for a USG for abdomen and scrotum. Conclusion- 1 seminal vesculitis 2 Cyst in right epididymis I consulted with doctor and he said that cyst is not harmfull and does not require any medication but he said to semen culture. In semen culture <link removed>li. bacteria was found and then doctor stopped all medicine and put me in Antibiotic(Doxy.lb and Septran Ds BD) For 21 days. After 21 days semen was analysed again and this time there was no bacteria but numbness was as before and some mild pain.now doctor again started Alfuzosin+Tofisopam and said after one month stop all medicine and said after 5 or 6month you will be ok. Now i finally want to say that the pain in testes and pelvic area is not major conceren as it is resolved. From past 10 days numbness also gone but itching in urethra is started I consulted another urologist he gave me Pregabalin+amitriptylline for one month. Now again a new problem is when itching is more the Skin if penis become very Loose also the muscle in penis Plz suggest me some some treatment .

Fill a tub with warm water (not hot).

Add: 1 tbsp Triphala powder (or) 2 tbsp Epsom salt (magnesium sulfate) Sit in the tub for 15–20 minutes, twice daily.

Thanks mam for your valuable time and advise. I understand that penis skin is elastic but in my case it is more elastic and not only the skin but also the muscle/penis shaft has become very loose. Some time i feel like some thing is in urethra like Ant is walking

It’s completely normal for the penis skin to paper loose or more wrinkled when flacid. This skin is highly elastic to accommodate changes in size between flaccid and erect states. If there’s no pain, swelling or change in skin texture or color, it’s likely not a medical concern

But in your case, you are having -discomfort or sensationa changes -a feeling of discomfort or numbness

then it may be related to pudendal nerve issues especially if this is part of CPPS or pudendal neuralgia

AND YES the treatment overlap for both, but not exactly the same

DIET AND LIFESTYLE

DIET -light, digestible food -moong dal, red rice, gourds, green vegetables -avoid spicy, oily, fried, and sour foods -drink warm water, barley water and coconut water -avoid alcohol, tobacco, and excessive sexual activity/masturation

LIFESTYLE -daily gentle yoga especially pawanmuktasana, baddha konasana, vajrasana -avoid sitting for long hours, take breaks if you work at desk -stress management -try bhramari, sheetali, anulom vilom
