Doctors’ responses
Hello. Stop stale junk foods and dry snacks. Do yogasana and kigle excercises. Tab chandraprabha vati 2 tabs after meals. Syrup Neeri KFT 2 tsp 2 times a Day with water. 2 tabs Shankh vati at night.
Hi Randeep, Your condition is a chronic pelvic pain syndrome. Urethral itching are also related to the post infectious features.
Along with some medication and lifestyle modification you get relieve from your symptoms.
1. Punarnavadi kashaya 15 ml+ 45 ml lukewarm water twice daily before food. ( To reduce any inflammatory conditions)
2. Gokshuradi guggulu 1-0-1 after food. ( Supports prostate and urinary discomfort)
3. Chandraprabha vati 1-1-1 after food. ( To relieve urinary tract infection and nerve irritation)
4. Triphala water sitz bath for 15 minutes in alternative days.
🛑Avoid prolonged sitting. Avoid spicy, acidic foods, caffeine and alcohol.
Do some streching exercises and yoga postures ( Setu bandhasana and suptha badha konasana are some yoga postures which strengthen the pelvic muscles )
Warm regards, Dr. Shaniba
Hi Randeep have you gone through any bloodtest/Urine examination /Any scanning, if yes pleased share the details. Meanwhile you can start, 1.Guluchyadi kwatham tab 2-0-2 before food 2.Chandraprabha gulika 2-2-2 after food 3.Gokshuradi guggulu 1-0-1after food
*Do’s 3-4litres of water /day More focus on fruits and vegetables Include Sprouted grains Have Tender coconut water Practice yoga and meditation regularly
*Don’ts Tea /coffee Oily too salty sour sweet foods Junk foods Carbonated/soft drinks Maida and its products
Hi Randeep, pain in testes is caused by several reasons which includes Varicocele, Hydrocele, Epidydimitis , Some testicular injuries, etc , and penile numbness is caused by some nerve damage /or in severe infections like STI ,Conditions like diabetes neuropathy, etc, since your diagnostic reports are normal there is nothing to worry about those . So please start the medication . May be a mild infection can cause these irritations
Yes i have done blood, urine and semen culture. All are in normal range I have done Ultrasound of abdomen and scrotum. This was also normal
OK so nothing to worry, please start the medication as I mentioned above
I want to know what is this condition called No body is telling me that even i contacted urologists
Medicine you prescribed 1.Guluchyadi kwatham tab 2-0-2 before food 2.Chandraprabha gulika 2-2-2 after food 3.Gokshuradi guggulu 1-0-1after food Do they work for nerve damage
I don’t think you have nerve damage, some inflammation/edema/Swelling around the nerves is causing this numbness. So we have to treat this inflammation, for that this set of medication is enough, follow this set of medicines for 2 weeks. Then you can have * Balaguluchyadi kwatham tab (kottakkal) 2-0-2 after food for 2weeks - 1month (For nerve health )
Yes i have done Initial analysis was Cyst in right epididyms Seminal vesculitis Further tested for semen culture and found E.Coli Doctor said that cyst is not harmfull And for <link removed>li doctor gave me 21days antibitotic treatment and again tetes the semen and found that the <link removed>li is not present in semen now
Ok You can start on Chandraprabha vati Gokahuradi guggulu Kanchanar guggulu- One tablet each twice daily after food with warm water
hello randeep, What you’re describing urethral itching, pelvic and testicular pain, penile numbness, and frequent urination that isn’t constant is classic of a condition called Chronic Pelvic Pain Syndrome (CPPS) or non-bacterial chronic prostatitis. This is not an infection but a nerve-muscle-inflammation problem in the pelvic area.
You got some relief from Alfuzosin (for urinary muscle relaxation) and Amitriptyline + Pregabalin (for nerve desensitization) this confirms the issue is linked to pelvic nerve irritation, congestion, and vata imbalance, not just infection.
In Ayurveda, this is mainly a Vata disorder, with some Pitta involvement. Nerves in the pelvic area are overstimulated, blood flow is poor, and there’s muscle tension that causes the discomfort and numbness. The condition can be reversed by calming Vata, improving circulation, and reducing inflammation.
Internal Medicines (6–8 weeks) Chandraprabha Vati – 2 tabs twice daily after food Gokshuradi Guggulu – 2 tabs twice daily after food Punarnavadi Kashayam – 15 ml + 45 ml warm water before food, twice daily Shilajit capsule (pure) – 300 mg in the morning with warm milk Bala churna – ½ tsp at night with milk (for nerve strength)
External Therapies (Do at Home or Nearby Clinic) Abhyanga (massage) with Mahanarayana Taila or Bala Taila on lower abdomen, back, thighs daily Hot Sitz Bath with a pinch of Triphala or Saindhava (rock salt) for 10 mins every evening
If possible, do Basti therapy (medicated enema) with Sahacharadi taila – 5 times alternate days (at clinic) Diet and Daily Tips Include:
Warm water all day Cow ghee – 1 tsp daily in meals Light food – lauki, moong dal, methi, jeera, ajwain Coconut water, pumpkin seeds Do Bhramari and Anuloma Viloma pranayama daily for 10 mins Avoid sitting long hours – get up every 30–40 minutes and walk Avoid strictly:
Curd, sour food, fried, spicy or cold drinks Late night sleep Skipping breakfast or long fasting Tight undergarments and long hours of sitting
Investigations to Confirm Urine Routine and Culture Ultrasound KUB with Prostate PSA (to rule out chronic inflammation) Semen analysis (if planning for fertility) Vitamin D3, B12, TSH Pelvic MRI (if symptoms persist more than 6 months)
You are on the right track. Medicines will help, but real recovery happens when you improve blood flow, nerve strength, and reduce tension from within. This is a fully manageable condition with the right approach.
If you have any doubts, you can contact me. Take care, Regards, Dr. Karthika
Thanks One thing i forgot to mention is that the looseness in penis skin is not in erect position This happens only in Flaccid position.
Is treatment goal for Pdeundal and CPPS are same
Is there chances that it can be due to pdeundal neurologia If yes how to conform that it is pdeundal or Prostatitis
Hello Randeep,
From what you’ve described itching in the urethra, pain in the testes and pelvic area, numbness in the penis, and frequent urination the symptoms are strongly suggestive of Chronic Prostatitis or CPPS (Chronic Pelvic Pain Syndrome). Since your relief comes only while taking medications like Alfuzosin, Amitriptyline, and Pregabalin, and symptoms return once stopped, it points to a condition that is more functional than structural likely nerve and inflammation-related.
Sometimes, this can also overlap with Pudendal Neuralgia, especially if the numbness and pain increase while sitting and reduce while standing or lying. Anxiety, sitting for long hours, or constipation can worsen this, as it compresses nerves in the pelvic region.
To confirm, you can:
Get a Pelvic Ultrasound or TRUS (Transrectal Ultrasound) to rule out prostate enlargement or congestion. Do a PSA test (though at your age, it’s usually normal). A Urine Culture + Urine Routine to rule out infection. If possible, a Pelvic MRI or Pelvic Floor Physiotherapy evaluation to assess any nerve entrapment or muscle tightness.
Sometimes, a Semen culture is advised if there’s sexual discomfort. Many patients with your exact symptoms often go undiagnosed or are labeled as “psychological” or “stress-related” — but the truth is, it’s a real condition involving pelvic nerve irritation and chronic inflammation, and yes, it can be reversed with the right approach.
If you have any doubts, you can contact me. Take care, Regards, Dr. Karthika
In the begining when numbness started i went to a urologist and told my situation. Doctor said that it is chronic prostatitis and prescribed medicine(Alfuzosin and Amitriptylline) But there was no relief Again he added some antibiotic and done prostatitis massage for 1 week on alternate days. Then i felt some relief in pain only . Again he continued same medicine. During this treatment i felt relief in urethra itching when i took Amitriptylline and now the pain was also reduced but numbness was as before . Now i went for a USG for abdomen and scrotum. Conclusion- 1 seminal vesculitis 2 Cyst in right epididymis I consulted with doctor and he said that cyst is not harmfull and does not require any medication but he said to semen culture. In semen culture <link removed>li. bacteria was found and then doctor stopped all medicine and put me in Antibiotic(Doxy.lb and Septran Ds BD) For 21 days. After 21 days semen was analysed again and this time there was no bacteria but numbness was as before and some mild pain.now doctor again started Alfuzosin+Tofisopam and said after one month stop all medicine and said after 5 or 6month you will be ok. Now i finally want to say that the pain in testes and pelvic area is not major conceren as it is resolved. From past 10 days numbness also gone but itching in urethra is started I consulted another urologist he gave me Pregabalin+amitriptylline for one month. Now again a new problem is when itching is more the Skin if penis become very Loose also the muscle in penis Plz suggest me some some treatment .

SYMPTOMS -pain in pelvis, perineum, testicles - burning or itching in urethra - increased urinary frequency - no bacteria in urine culture - erectile issues sometimes -discomfort relieves with muscle relaxants or alpha blockers
2) PUDENDAL NEURALGIA A nerve pain condition caused by compression or irritation of the pudendal nerve, which supplies the genitals, anus and perineum
SYMPTOMS -burning, numbness, pins and needles in penis, scrotum, perineum -pain worse while sitting - urinary frequency or urgency - no relief with antibiotics -better with nerve pain meds like pregabalin, Amitriptyline
LETS DIFFRENTIATE BOTH
PAIN in prostatitis= deep pelvic/testicular pain Pudendal neuralgia= sharp, burning, stabbing
NUMBNESS prostatitis=rare pudendal neurallgia= common on penis/ perineum
SITTING prostatits= usually okay pudendal neuralgia = worsens pain
URINATION prostatitis= frequency, urgency pudendal neuralgia= may be present
EXAMINATION prostatitis= tender prostate pudendal neuralgia= tende pudendal nerve on rectal touch
INVESTIGATIONS prostatitis= urine culture, prostate exam, semen analysis pudendal neuralgia= MRI pelvis(nerve compression), nerve conduction study
MENTION IF ANY OF THE TEST ALREADY DONE AND WHAT IS THE INTERPRETATION
ACCORDING TO AYURVEDA,
your symptoms suggest a disorder of vata dosha, particularly Apana Vata which governs the pelvic region, bladder and reproductive system
ALSO INVOLVED -pitta aggravation= causing burning, itching, inflammation -Mutravaha srotas dushti= disturbance in urinary system channels -Shukravaha srotas dushti= disorder in reproductive channels-testicular pain
so likely it is a vata pita predominant condition, involving mutravaha and sukravaha srotas
TREATMENT GOALS -pacify vata and pitta -strengthen nerves and dhatus - correct mutravaha and shukravaha srotas -avoid recurrence through rejuvination

HELLO RANDEEP,
Thank you for sharing your detailed medical journey- you have been clearly been through a lot, and it’s understandable to want lasting relief after dealing with pain, numbness, and now recurring urethral itching.
You’ve already had thorough allopathic care- including antibiotics, alpha-blockers , antidepressants/nerve stabiliser(like Amitriptyline and pregabalin), and even prostatitis massage. It’s good that pain and numbness are mostly resolved, but the recurring urethral itching and feeling of loose penile skin/muscle now seem to be primary concern.
Fron an ayurvedic perspective, your condition fall under a few overlapping categories -MUTRAKRICHRA= difficulty in urination or discomfort in urinary tract - SHUKRA VAHA SROTAS DUSHTI= disorder of reproductive channels - Possibly Vata-pitta dosha imbalance as I have mentioned earllier
LETS GO FOR SYMPTOMATIC TREATMENT
FOR ITCHING AND URINARY TRACT HEALTH
1) CHANDRAPRABHA VATI= 2 tabs twice daily after meals with lukewarm water
2) GOKSHURADI GUGGULU= 2 tabs twice daily after meals with warm water

FOR NERVE WEAKNESS AND NUMBNESS
4) ASHWAGANDHA CAPSULES= 1 cap twice daily with warm milk in morning and night
5) BALA TAILA= 10 drops with milk after meals
C) FOR REPRODUCTIVE TISSUE NOURISHMENT
6) SHATAVARI KALPA OR SHILAJIT RASAYAN= if sexual weakness is a concern
7) KAUNCH BEEJ CHURNA= 1 tsp in morning and night with honey =improves nerve tone
LOCAL APPLICATION
- YASHTIMADHU TAILA= gentle local massage around groin/ penis shaft not inside urethra
-WARM SITZ BATH with TRIPHALA DECOCTION AND NEEM LEAVES= daily 15-20 minutes
AVOID ALCOHOL, MASTURBATION AND EXCESSIVE SITTING
AND FOLLOW ABOVE MENTIONED EXERCISE YOGA AND DIET STRICTLY
DO THIS FOLLOW THIS AND TRUST ME USE ALL THIS FOR 1-2 MONTHS AND YOU WILL SEE IMPROVEMENT IN YOUR CONDITION AND THEN THANK ME LATER
THANK YOU
DR. MAITRI ACHARYA
It’s completely normal for the penis skin to paper loose or more wrinkled when flacid. This skin is highly elastic to accommodate changes in size between flaccid and erect states. If there’s no pain, swelling or change in skin texture or color, it’s likely not a medical concern
But in your case, you are having -discomfort or sensationa changes -a feeling of discomfort or numbness
then it may be related to pudendal nerve issues especially if this is part of CPPS or pudendal neuralgia
AND YES the treatment overlap for both, but not exactly the same
But the treatment I had given will work for the symptoms you are facing and will cure your problem in short span of time with deep healing
so don’t take stress much and take treatment as advised for early healing
THANK YOU
DR. MAITRI ACHARYA
Your symptoms such as itching in urethra, pain in the testes and pelvic area, penile numbness and frequent urination could be related to the condition known as chronic prostatitis or possible urinary tract infection. While it’s essential to consult with a urologist or a healthcare professional for an accurate diagnosis, Ayurveda offers holistic strategies that could assist in managing your condition by addressing underlying dosha imbalances.
In Ayurveda, these symptoms suggest a possible imbalance of Vata and Pitta doshas. Vata can contribute to pain and discomfort while Pitta can result in inflammation and burning sensations. Here are several steps to help you:
1. Dietary Adjustments: Favor warm, easily digestible foods. Reduce intake of spicy, pungent, or sour foods which can aggravate Pitta. Include ghee, milk, and cooling items such as cucumber and aloe vera to balance Pitta. Avoid caffeine and alcohol.
2. Hydration: Drinking plenty of warm water can help maintain urinary health and assist in flushing out toxins. You can infuse water with coriander seeds or fresh mint—good for reducing Pitta.
3. Lifestyle Practices: Gentle yoga poses, like bhujangasana (cobra pose) and supta baddha konasana (reclined bound angle pose), can help alleviate pelvic tension. Practice daily for optimal results. Evening walks (if comfortable) can promote circulation.
4. Herbal Remedies: Consider using Gokshura (Tribulus terrestris) and Punarnava (Boerhavia diffusa), traditionally used for urinary system support. Ayurvedic preparations like Chandanasava can be soothing for inflammation. Always consult with an ayurvedic physician for proper dosage.
5. Mindfulness & Stress Management: Stress can exacerbate symptoms. Practices such as meditation and Pranayama (breathing exercises) are beneficial. Focus on regular sleep patterns.
Remember, these are supportive steps, not substitutes for medical interventions. If symptoms persist or intensify, seeking prompt consultation with a health care provider is necessary. Ayurveda offers an integrative approach but should complement rather than replace urgent medical treatment.
Sir, you do kegel exercises 2-3 times daily Apply ashwagandha bala/ Shilajit oil on penis externally twice daily Take cap. Ashwashila 1-0-1 after food with water Follow up after 1 month
Based on the symptoms you’ve described, it sounds consistent with a condition that could involve both infectious and non-infectious components like chronic prostatitis or interstitial cystitis. Your current medications address symptoms such as urinary difficulties and neuropathic pain, which explains the relief experienced. However, since these symptoms persist, thorough examination of underlying causes through Ayuvedic lens might be beneficial.
In the Siddha-Ayurvedic perspective, these symptoms could be due to an imbalance in the apan vayu, one of the sub-doshas of vata, which is responsible for the elimination process in the body. This can be influenced by factors like stress, diet, and lifestyle. To address this, integrating dietary modifications and specific treatments can be helpful.
1. Focus on a Vata-pacifying diet: Incorporate warm, cooked foods. Avoid cold and raw foods which can aggravate vata. Include ghee, which can help in lubricating the tissues.
2. Herbal Support: Consider taking Gokshura (Tribulus terrestris) which can support urinary system health. It helps reduce swelling and irritation specifically in the lower urinary tract.
3. Abhyanga (oil massage): Daily self-massage with warm sesame oil can help in soothing the vata and aid in relaxing the pelvic region.
4. Yoga & Pranayama: Gentle yoga exercises focusing on pelvic stretches can improve blood flow in the area affected. Coupled with breathing exercises like Anulom Vilom to calm the nervous system.
These interventions target root imbalances beyond symptom relief. However, please consult a qualified healthcare provider to rule out any serious underlying conditions and before making drastic lifestyle changes. If symptoms persist or worsen, seek immediate medical attention for a detailed evaluation and appropriate management.
HELLO RANDEEP,
In Ayurveda prostatitis is closely related to vata dosha aggravation in the apana vata region (pelvis/lower abdomen), often coupled with pitta imbalance (inflammation, burning sensation) and sometimes kapha(blockage or swelling)
The condition can ve viewed as a mix of -mutraghata (urinary obstruction) -SHOTHA- prostate gland inflammation
ADVISED PANCHAKARMA FOR DEEP DETOX AND HEALIN -BASTI- medicated enema -ABHYANGA- oil massage -SWEDANA- to. reduce swelling and pain -UTTARA BASTI-administered through urethra=very powerful and effective
DIET AND LIFESTYLE
DIET -light, digestible food -moong dal, red rice, gourds, green vegetables -avoid spicy, oily, fried, and sour foods -drink warm water, barley water and coconut water -avoid alcohol, tobacco, and excessive sexual activity/masturation
LIFESTYLE -daily gentle yoga especially pawanmuktasana, baddha konasana, vajrasana -avoid sitting for long hours, take breaks if you work at desk -stress management -try bhramari, sheetali, anulom vilom
INTERNAL MEDICATIONS
1) BALA TAILA -internal= 10 drops with warm milk -external= gentle massage over lower abdomen and lower back =recues Vata in pelvic region, strengthens reproductive organs
2) ASHWAGANDHADI LEHYA -1 tsp daily twice with warm milk =chronic pain, weakness, sexual side effects of prostatitis
3) BASANT KUSUMAKAR RAS= 125 mg once daily with milk =strengthen urogenital system
4) TRIPHALA GUGGULU= 2 tabs twice daily after meals =sweeling, constipation related pressure on prostate
