Hello, Your sister (46 years) has type 2 diabetes with persistently high fasting blood sugar (>150 mg/dL) despite disciplined efforts—Ayurvedic medicines (Setebid, Amrita Mehari Churna), regular walking after meals, millet-based diet, methi water, and she is also on thyroxine for hypothyroidism and Setebid for high cholesterol.

This suggests early-morning hepatic glucose overproduction, insulin resistance, and associated hypothyroidism, which commonly causes fasting sugar to remain high even when post-meal control is better.

🔎 Recommended Investigations (If not done in last 3 months) Fasting & PP blood sugar HbA1c Fasting insulin (if available) Lipid profile TSH (ensure hypothyroid is well controlled) LFT (before long-term medicines)

💊 Internal Medicines Phase 1 – Correct Fasting Sugar & Insulin Resistance (45 days) (Night-time liver glucose control + Agni correction) 1.Nishamalaki Churna – ½ tsp with warm water after dinner 2.Chandraprabha Vati – 2 tablets twice daily after meals 3.Vasant Kusumakar Rasa (plain) – 1 tablet once daily after breakfast

Phase 2 – Long-term Stabilization (90 days) 1.Katakakhadiradi Kashayam – 15 ml + 30 ml warm water, twice daily before food 2.Triphala Guggulu – 2 tablets at bedtime 3.Shilajit Capsule (standardized) – 1 capsule in the morning after food

🌿 Supportive Home Remedies Fenugreek (Methi) – continue, but: • 1 tsp soaked overnight • Chew seeds, avoid excess water Bedtime drink (very important) • ½ tsp turmeric + pinch of black pepper • In warm water, after dinner

🥗 Diet Modifications (Important corrections) ❌ Common Mistakes to Avoid Only millets all day (can raise fasting sugar if overused) Dinner too late Fruits at night Excess raw salads at night ✅ Correct Diet Plan • Breakfast: millet + protein (dal / curd if tolerated) • Lunch: millet + vegetables + dal • Dinner (before 7:30 pm): → vegetable soup + cooked vegetables • Add 1 tsp cow ghee/day (improves insulin response) 👉 Ensure adequate protein, not only vegetables.

🚶🏻‍♀️ Exercise & Lifestyle ✔ Continue walking after meals ✔ Add 10 minutes slow walk after dinner ✔ Avoid late nights (sleep before 10:30 pm) ✔ Stress management (stress elevates fasting sugar)

🧘🏻‍♀️ Yoga & Pranayama Asanas • Vajrasana (10 min after meals) • Pawanmuktasana • Ardha Matsyendrasana Pranayama Kapalbhati – 30–50 strokes × 2 rounds (if BP normal) Anulom Vilom – 10 minutes daily

🕉️ Prognosis & Follow-up • Fasting sugar reduction: 3–4 weeks • HbA1c improvement: 3 months • Thyroid balance helps sugar control—ensure TSH is optimal • Consistency > number of medicines

✨ Persistently high fasting sugar needs night-time correction and liver-focused therapy, not just daytime diet control.

With kind regards, Dr. Sumi MS (Ayu)

WHEN FASTING SUGAR REMAINS HIGH DESPITE WALKING DIET CONTROL AND MEDICINES IT MEANS THE PROBLEM IS NOT ONLY FOOD RELATED IT IS ALSO DUE TO NIGHT TIME LIVER GLUCOSE RELEASE HORMONAL IMBALANCE AND DIGESTIVE METABOLIC DISTURBANCE

YOUR SISTER IS DOING MANY RIGHT THINGS WALKING AFTER MEALS,LIMITED DIET,METHI WATER AND TAKING MEDICINES

STILL FASTING SUGAR IS ABOVE 150 SUGGESTS NIGHT TIME GLUCOSE DUMP FROM LIVER,WEAK AGNI AND METABOLISM ,STRESS OR SLEEP DISTURBANCE,THYROID AND DIABETES INTERACTION

SETABID AND AMRITA MEHARI CHURNA CAN HELP BUT THEY MAINLY WORK ON POST MEAL SUGAR NOT FASTING

FOR FASTING SUGAR CONTROL THESE ARE IMPORTANT TAKE NISHA AMALAKI CHURNA HALF TEASPOON MORNING EMPTY STOMACH WITH LUKEWARM WATER THIS HELPS LIVER FUNCTION AND FASTING SUGAR TAKE CHANDRAPRABHA VATI ONE TABLET TWICE DAILY AFTER BREAKFAST AND DINNER THIS IMPROVES METABOLISM URINARY SYSTEM AND HORMONAL BALANCE TAKE GUDMAR CHURNA HALF TEASPOON CHURNA BEFORE DINNER THIS REDUCES NIGHT TIME GLUCOSE RELEASE

CONTINUE AMRITA MEHARI CHURNA ONLY MILLETS AND VEGETABLES CAN SOMETIMES INCREASE FASTING SUGAR BECAUSE TOO LOW CARBOHYDRATE AT NIGHT MAKES LIVER RELEASE MORE GLUCOSE

DINNER SHOULD BE LIGHT AND BEFORE 7 PM AND INCLUDE MOONG DAL OR VEGETABLE SOUP SMALL AMOUNT OF RICE OR JOWAR ROTI,COOKED VEGETABLES AVOID RAW SALAD AT NIGHT

AVOID MILLETS AT NIGHT MILLET ARE HEAVY AND DRY THEY DISTURB VATA AND LIVER FUNCTION

SLEEP BEFORE 10 PM LATE NIGHT SLEEP DIRECTLY INCREASES FASTING SUGAR

AVOID MOBILE AND TV AFTER 9 PM

5 MINUTES OF DEEP BREATHING BEFORE SLEEP

THYROID MEDICINE MUST BE TAKEN EARLY MORNING EMPTY STOMACH NO FOOD FOR AT LEAST 45 MINUTES AFTER THYROXINE

IRON CALCIUM OR AYURVEDIC POWDERS SHOULD NOT BE TAKEN CLOSE TO THYROID MEDICINE

POOR THYROID CONTROL CAN KEEP FASTING SUGAR HIGH THYROID LEVELS SHOULD BE CHECKED EVERY 6 MONTHS

WITH THESE CHANGES FASTING SUGAR CAN START IMPROVING IN 3 TO 4 WEEKS

REGULAR MONITORING IS IMPORTANT

DO NOT STOP ALLOPATHIC MEDICINES SUDDENLY AYURVEDA WORKS BY CORRECTING ROOT CAUSE AND NEEDS CONSISTENCY

1.Tab. Diabecon DS 2 tab twice daily 30 min before meals with water 2.Nisha Amalaki churna 1 tsp twice daily with water before meals

Continue Amrita Mehri churnam…

🌸 Supportive Routine - Evening meal: Keep it light and early (before 7 pm). Heavy dinners often raise fasting sugar. - Bedtime support: A small piece of cinnamon or fenugreek seeds soaked in warm water can help overnight sugar control. - Hydration: Warm water sips through the day; avoid cold water. - Yoga/Pranayama: Kapalabhati (gentle, if tolerated), Anulom Vilom, and Surya Namaskar in the morning improve metabolism. - Skin & thyroid care: Since she is on thyroxine, avoid excess goitrogenic foods (raw cruciferous vegetables).

It suggests insulin resistance + possible under-treated hypothyroidism which is common in women this age.

Check thyroid levels – repeat TSH, free T4 (hypothyroidism not fully controlled raises sugar). Discuss dose increase with doctor.

HbA1c & fasting insulin test – to assess resistance.

Add these Ayurvedic Medicines 1 BGR-34 OR Madhunashini Vati – 2 tablets twice daily 30 min before meals 2 Karela + Amla juice – 20 ml each morning empty stomach 3 Vijaysar Churna – 3 gm night with warm water 4 Shilajit resin (pure) – 300 mg morning with warm water

Diet- Breakfast: add protein – 2 moong chilla OR vegetable oats + 1 tsp ghee Continue millet/veg but add moong dal + ghee in meals Night: warm milk + pinch turmeric (no carbs after 7 PM) Continue methi water

Lifestyle Increase walk to 30–45 min after dinner -Sleep 10 PM – 6 AM strictly (poor sleep raises morning sugar) -10 min Anulom-Vilom daily

HELLO,

Your sister has diabetes along with hypothyroidism and high cholestrol. In Ayurveda, these are not separate diseases - they are connected problems of slow metabolism

WHY FASTING SUGAR IS HIGH EVEN AFTER DIET AND WALKING? Think of the body like a factory -Food-> digestion->energyy->storage-> use -In her case -digestion is slow -fat metabolism is weak -liver releases extra sugar at night -thyroid weakness slows everything further

So even if she eats well and walks the body -makes sugar internally at night -cannot burn stored fat and sugar properly -stores more instead of using it

This is why fasting sugar stays above 150.

AYURVEDIC VIEW -Madhumeha (type of prameha) -Kapha-dominant with meda (fat tissue) involvement -Agnimandya - weak digestive and metabolic fire -Yakrit (liver) dushti- excess glucose relase -Avarana by kapha- blockage of insulin action -Associated ras and meda dhatu dushti -Hypothyroidism worsens all the above

TREATMENT GOALS -reduce fasting sugar naturally -improve liver glucose control -correct fat metabolism -strengthen digestive and tisue metabolism -support thyroid function -prevent complications (nerve, kidney, heart)

INTERNAL MEDICATIONS

A) FOR BLOOD SUGAR CONTROL

-NISHAMALAKI CHURNA= 5 gm at bedtime with warm water minimum 3-6 months =turmeric improves insulin sensitivity, amalaki strengthens liver and pancreas, especially helpful for fasting sugar

-GUDMAR CAPSULES= 1 cap twice daily before meals for 3-6 months =reduces sugar absorption, improves pancreatic function, reduces sugar cravings

-VIJAYSAR CHURNA= 3 gm twice daily with warm water after meals =classical anti-diabetic drug , improves insulin action, controls fasting and post meal sugar

B) FOR CHOLESTROL + FAT METABOLISM

-TRIPHALA GUGGULU= 2 tabs twice daily after meals for 4 months =reduces fat accumulation , clears blocked channels, supports weight and lipid reduction

C) FOR THYROID SUPPORT

-KANCHANAR GUGGULU= 2 tabs twice daily after meals for 3 months =improves thyroid gland function, reduces kapha stagnation, helpful in hypothyroidism and lipid disorder

IMPORTANT RULE -Maintain 45-60 min gap between Thyroxine and Any Ayurvedic medicine

YOGA ASANAS -vajrasana -mandukasana -ardha matsyendrasana -paschimottanasana -setu bandhasana

PRANAYAM -Kapalbhati= 20-30 strokes * 3 months -Bhastrika= improves metabolism -Anulom vilom= hormonal balance -Bhramari= stress control

Practice daily, empty stomach

DIET -old rice -barley -moong dal -bitter vegetables -lauki, pumpkin, ridge gourd -cow ghee small amount

AVOID -fruits at night -milk and curd at night -banana, mango, chikoo -refined flour and sugar -fried foods

MILLETS -Good but not daily and not at night. They are heavy and worsen fasting sugar if misused

HOME REMEDIES -Methi water= continue -Cinnamon 1/2 tsp + warm water morning -Soaked almonds 4-5 daily -bitter gourd juice twice a week

LIFESTYLE ADVICE -Dinner before 7 pm -sleep by 10 pm -no day sleep - daily walking is good- continue -stress reduction is essential - stress increases sugar

VERY IMPORTANT -Fasting sugar does not correct overnight -Ayurveda works slowly but deeply -consistency is more important than quantity of emdicines -Thyroid + diabetes must be treated together -Never stop modern medicines suddenly

DO FOLLOW

HOPE THIS MIGHT BE HELPFUL

THANK YOU

DR. MAITRI ACHARYA
