This is a very long standing constipation and not a simple digestion issue When stools are soft but still difficult to pass it usually means dryness and weakness of the rectum and anal canal which is a vata dominant condition
That is why you feel the urge many times 4 to 5 visits but only a small amount comes out with straining Occasional Triphala does not help in such old cases because it stimulates the bowel but does not provide lubrication or nerve strength
This condition needs lubrication and vata balancing not strong laxatives
What you can do is Take 1 teaspoon of pure ghee daily at morning with warm water Erandbrisht Haritaki -1 teaspoon with warm water can be taken at night
Avoid straining Go only when the urge is natural
Eat warm cooked food Avoid dry foods like biscuits bread toast Avoid curd at night
As your water intake is adequate so this is not dehydration It is internal dryness developed over many years
