To address your inflammatory disease, especially the persistent back and neck pain, we’ll delve into some Ayurvedic insights to complement your current allopathic treatments. Given the complexity of your symptoms—a combination of pain, fatty liver, and sugar imbalances—ayurvedic approach should be gradual and holistic.

First off, addressing your dosha imbalance is crucial. Vata dosha, when aggravated, often manifests as pain, especially in the back and neck. To pacify Vata, incorporate warming, grounding, and nourishing foods. Consider introducing a diet rich in cooked vegetables, whole grains such as quinoa or brown rice, and fats like ghee or sesame oil. Avoid raw and cold foods which could increase Vata.

Since you’re dealing with a fatty liver, it’s important to support your liver function. Drinking a glass of warm water with half a lemon squeezed added first thing in the morning can stimulate liver function. Turmeric is another excellent choice; adding it to your meals or taking it as a supplement can aid in reducing inflammation.

Managing blood sugar levels calls for a careful dietary balance. Foods with a low glycemic index, like leafy greens, berries, and lentils, are ideal. Stay away from refined sugars and processed foods. Sprinkle a teaspoon of fenugreek seeds soaked overnight in water on your meals could help regulate sugar levels.

For external application, a simple sesame oil massage to affected areas can help reduce pain and increase circulation, just ensure it’s warm before applying. Do this at least three times a week, followed by a warm bath, can be beneficial.

Incorporate gentle yoga and pranayama into your routine to help with stiffness and stress. Natural movements and breathwork, like those found in basic surya namaskar or anulom vilom, can enhance your overall well-being without putting strain on your back and neck.

Remember to consult with a local Ayurvedic practitioner in order to tailor these recommendations based on your specific needs. Your condition needs comprehensive management, including regular check-ups with your allopathy practitioner to monitor any changes and adjust medications as necessary. Safety first, so ensure that the Ayurvedic methods complement and do not interfere with your existing medical regime.
