Focal Hand Dystonia in the right Index Finger - #25938

I am 63, a Tabla soloist. I had right hand Frozen Shoulder thrice during the last 10 years period and all incidents cured by Ayurvedic treatment. Five years back, without any symptoms, my right hand index fingers curl inward while playing Tabla. I tried Ayurvedic treatment but in vein. Later, neurologists diagnosed it as Focal Hand Dystonia of Right Hand. Now, I am unable to play on the instrument due to spasm and curling of the right hand finger. I would be grateful if you could advice.

Hello, 1. Occupational therapy/physiotherapy 2. Massage the finger with mahanarayana thailam followed by dhara with balamula kashayam. Total duration-20 minutes everyday.

Start by incorporating a Vata-pacifying diet, which includes warm, moist, and nourishing foods. Focus on cooked vegetables like carrots, beets, and squash, grains like rice and wheat, and proteins like dhal and mung beans. Avoid cold and dry foods that aggravate Vata, such as raw salads, crackers, and excessive caffeine.

In terms of lifestyle, consider incorporating Abhyanga, or self-massage with warm sesame oil, to your daily routine. This can help relax the muscles and improve circulation. Massage should be followed by a warm shower to further soothe the muscles. Practicing Pranayama, particularly Nadi Shodhana (alternate nostril breathing), can help calm the nervous system and balance Vata.

Herbs that support the nervous system could also be beneficial. Ashwagandha, with its adaptogenic properties, helps in reducing stress and restoring balance. Consider a daily dose of Ashwagandha powder with warm milk, though it’s advisable to consult with a local Ayurvedic practitioner for proper dosages specific to your constitution.

For therapeutic treatments, Shirodhara can help relax the mind and body, by pouring warm oil in a steady stream over your forehead, providing deep relaxation and restoring nervous system balance. FOCAL HAND DYSTONIA- also called “musician’s dystonia”- is a task-specific movement disorder where involuntary muscle contractions cause abnormal postures or movements. In musicians, especially percussionists like table players, it commonly affects fingers or hands used for precise repetitive movements. Since you mentioned that it affects your right index finger while playing, this is consistent with classic FHD.

In Ayurveda, such conditions are often classified under “vata vyadhi”- disorders caused by the vitiation of vata dosha, which governs movement and nervous system function

LIKELY CAUSES -repetitice overuse - age related vata aggravation - prior history of frozen shoulder

SYMPTOMS -involuntary movements -muscle stiffness/ spasms - incoordination, especially during skilled tasks like table playing

NOTE= FHD is chronic and can be resistant to complete reversal- but symptoms can often be managed or improved with a combination of therapies

Ayurvedic treatment aims to restore balance of vata, nourish the nervous system, and reduce spasms

INTERNAL MEDICATIONS

ASHWAGANDHA CHURNA= 1 tsp with warm milk twice daily

BRAHMI GHRITA= 10 ml once daily in morning on an empty stomach

KSHEERBBALA 101 CAPSULES= 1 cap twice daily with warm milk

MAHA NARAYAN TAILA= 10 drops twice daily with warm milk

VATAGAJANKUSHA RASA= 250 mg twice daily with honey and ghee

EXTERNAL THERAPIES

OIL MASSAGE= with MAHANARAYAN TAILA= focus on shoulder, arm, hand

Localised steam therapy to reduce stiffness

NASYA= instill 2 drops of Anu taila in each nostril

DIET AND LIFESTYLE -warm, oily, nourishing foods avoid overuse of the hand, repetitive stress -meditation and pranayam= anulom vilom to reduce neurological tension

ADJUNCTIVE SUPPORT

HAND THERAPY AND MUSCLE RETRAINING= work with a physiotherapist who understands musicians dystonia

NEUROPLASTIC RETRAINING= mirror therapy or constraint- induced movement therapy may help
