How to treat my father's sciatica pain and improve his physical activity with diabetes? - #54397
My father has been suffering from sciatica for the past year and is unable to perform physical activities properly. He also has diabetes. Please suggest an appropriate solution. Doctor’s Assessment: Lumbar muscle spasm → Tight lower back muscles → Loss of normal lumbar curve L5 over S1 anterolisthesis (Grade I–II) → Mild forward slipping of L5 vertebra → Can cause back pain and leg pain Degenerative (wear & tear) changes → Small bone spurs (osteophytes) → Endplate changes at multiple levels Disc bulges → Present at L3–L4 → Present at L5–S1 → Mild indentation on spinal sac Nerve compression → At L3–L4 level → At L5–S1 level → Compression of exiting nerve roots → Can cause sciatica symptoms Disc tear at L5–S1 (annular tear) → Tear in outer disc layer → Possible disc extrusion → Major contributor to pain How would you describe the intensity of your father's sciatica pain?: - Mild — noticeable but manageable Has your father experienced any numbness or tingling in his legs?: - Yes, frequently What treatments has your father tried for his sciatica pain?: - Prescription medications How is your father's blood sugar control?: - Well-controlled How does your father describe his physical activity level?: - Very active Does your father have any other health conditions?: - No, just diabetes How would you rate your father's overall energy levels?: - Low — often tired
Doctors' responses
Your father’s MRI shows disc bulge + nerve compression + mild vertebral slip, which is a structural spine issue. So treatment should focus on reducing nerve pressure + improving stability, not just pain relief. What to do (practical plan) 1. Activity modification (very important) ❌ Avoid bending forward, lifting weight, sudden twisting ❌ Avoid long sitting ✔ Use firm bed for sleep ✔ Walk in short intervals (don’t overdo) 2. Physiotherapy (key treatment) Daily guided exercises: McKenzie extension exercises Core strengthening Hamstring stretching 👉 This is most important for long-term improvement 3. Pain & nerve care (Ayurveda support) Yograj Guggulu – 1 tablet twice daily after food Mahayograj Guggulu – 1 tablet twice daily after food Dashmool kwath – 15 ml twice daily 4. External therapy Daily warm oil massage (Mahanarayan oil) on lower back Followed by hot fomentation 5. Diabetes care Since he has diabetes: Avoid sugary formulations Keep sugar well-controlled → nerve healing improves 6. Ghar par Sleeping: pillow under knees (if lying on back) Sitting: lumbar support cushion When to consider advanced care Increasing weakness in leg Loss of bladder/bowel control Severe persistent pain 👉 Then spine specialist evaluation needed (injection/surgery options) Expectation Pain relief: 2–4 weeks Functional improvement: 6–8 weeks (with physiotherapy) Key point: 👉 Exercise + posture correction = main treatment Medicines are only support. If you want, I can give a day-wise exercise routine for him. Regards, Dr Raghuveer (Ayurvedacharya)
this condition is mainly seen as Vata aggravation in the lumbar region (Gridhrasi – Sciatica) along with Avarana (nerve obstruction) due to disc degeneration. With diabetes, we must choose mild, safe, and anti-inflammatory Vata-pacifying care. Ayurvedic Approach 1. Pain & Nerve Relief Daily warm oil massage (Abhyanga) with Mahanarayana oil or Dhanwantharam oil on lower back and legs Followed by hot fomentation ( warm towel heat) → Reduces stiffness, improves nerve flow 2. Internal Support (Vata + Nerve healing) Ashwagandha + Guggulu formulations Dashmool Kwath for inflammation and muscle relaxation Yograj Guggulu is often used in sciatica (only under supervision, especially with diabetes) 3. Physical Activity (Very important) Gentle walking 15–20 minutes twice daily Simple physiotherapy stretches (hamstring stretch, pelvic tilt) Avoid long sitting, bending, and lifting weights 4. Diabetes-friendly care Keep diet light: barley, green vegetables, bitter gourd, turmeric Avoid excess sugar, fried foods, and cold foods (they worsen Vata + nerves) 5. Local therapy (if available) Kati Basti (oil retention therapy on lower back) → very effective in disc + sciatica cases Basti (medicated enema therapy) → best for chronic Vata disorders
