Hello , Your knee has been gradually losing bone strength for the last 10 years. Because of your past cancer history, menopause, age, and extra weight, the bone cushioning and cartilage have thinned, which is why standing and walking for long periods feel difficult even if there is not much pain. The good news is: • Your knees are not bow-shaped, meaning the alignment is still good • You do not have severe pain this is a positive sign • Many women in your stage avoid surgery successfully by strengthening muscles, improving bone density, reducing inflammation, and reducing weight slowly

You do not need quick aggressive treatments your body needs steady, safe, supportive therapy.

2. Internal Ayurvedic Medicines

Start with Ama Pachana (first 7 days): • Amapachana Vati – 2 tablets twice daily after food for 7 days

After Day 7, continue for 2–3 months: • Asthi Shrinkhala (Hadjod) Capsule – 1 capsule twice daily after food • Punarnavadi Guggulu – 1 tablet twice daily after food • Shatavari Capsule – 1 capsule at bedtime • Ashwagandha Capsule – 1 capsule at bedtime • Tribhuvan Mritadi Kwath – 10 ml with equal warm water, twice daily

(All are safe for someone with your medical history—but avoid any herb with phytoestrogen in high dose; this combination is balanced.)

3. External Treatments (you can do at home) • Mahanarayan Taila massage to both knees daily for 10 minutes • After massage, apply warm towel fomentation for 5 minutes • Avoid strong heat or steam (bad for osteoporosis)

4. Exercises / Yoga (safe for osteoporosis & your age)

Do daily, 15 minutes: • Supported wall-squats (very shallow) – 5–8 repetitions • Straight Leg Raises • Quadriceps strengthening with towel under knee • Ankle rotations • Setu Bandhasana (Bridge pose) – gentle • Tadasana – improves posture

Avoid: • Fast walking • Sitting cross-legged • Jumping exercises • Deep squats

5. Diet (simple & practical) • Add: • 1 teaspoon ghee daily • Soaked almonds (5) + 1 date every morning • Moong dal, ragi, sesame seeds, paneer • Warm homemade food • Reduce: • Sugar • Deep fried food • Excess wheat • Packaged snacks

For weight loss (steady & safe): • Dinner before 7:00 PM • Soup or moong dal khichdi for dinner 3–4 times a week

6. Investigations You Should Do • Vitamin D3 • Vitamin B12 • Serum Calcium • CRP (inflammation) • Bone Density Scan (DEXA) – 1 time per year

These help track bone health without needing surgery.

7. Do’s & Don’ts

Do: • Use a knee support while walking • Take breaks while standing • Use soft footwear • Lose weight slowly (even 5–6 kg loss reduces knee load by 30%)

Don’t: • Sit on low chairs • Climb stairs repeatedly • Walk barefoot • Over-exercise when tired

You have lived through a major illness and hysterectomy already your body is strong and capable. Your knee condition is manageable with consistent care. You don’t have bowing of legs or severe pain, which means you still have a window to avoid knee replacement if you stay steady with treatment, diet, and exercises.

You are not alone, and with small daily steps, you will gain more comfort in walking and reduce stiffness.

Warm regards, Dr. Karthika

Hello‍‌‍‍‌‍‌‍‍‌ dear, Thank you for sharing your medical history with such honesty. I really respect and admire your bravery of going through all these alone — cancer of the endometrium in the past, being overweight, and now you have had knee degeneration for a very long time.

It is quite understandable to be doubtful about surgery when you do not have much help after the operation.

The positive thing is: you still can endure the pain, your legs are not twisted and you have already started doing exercises, therefore, there is a very good opportunity to slow down the disease, lessen the pressure on the knee, and increase your walking capacity without a knee replacement at this stage.

✅ Understanding Your Knee Problem in Ayurvedic Terms

The main aspects that led to the problem: –Age (58 years) → Natural increase of Vata –Overweight → Extra load on knee joint –History of hysterectomy → Long-term hormonal and tissue nourishment imbalance –10 years of degeneration → Chronic dryness and wear of cartilage

This accounts for: –Difficulty standing for long –Inability to walk for long distances –Progressive joint weakness without severe deformity yet

✅ VERY IMPORTANT REASSURANCE

Given that: –You do not suffer from severe pain –You do not have bowing of legs –You are still capable of exercising –Moreover, your condition has developed slowly over many years

👉 Conservative, non-surgical treatment will work very well in your case. A knee replacement is NOT the only option at the moment.

✅AYURVEDIC PLAN OF TREATMENT

Focus of treatment 1. Joint lubrication 2. Bone and cartilage strengthening 3. Body weight reduction 4. Circulation and mobility improvement

✅EXTERNAL THERAPIES

If there is qualified Panchakarma doctor near you who can perform these procedures:

👉 Balashwagandhadi Taila OR Ksheerabala Taila loaded Janu Basti→ Nourishes cartilage, reduces stiffness, delays degeneration

👉Abhyanga (Oil massage) of both legs daily at home: Utilize Mahanarayana Taila Massage knees, calves, and thighs gently before taking a bath After the massage, use warm water for fomentation

Just these can go a long way to improving your walking stamina within a period of 4–8 weeks.

✅ INTERNAL MEDICATION

1. Nucart OA 2-0-2 after food ( helps to improve cartilage in the joint ) 2. Shallaki MR 1-0-1 after food ( reduces pain) 3. Saharchadi kashayam 15ml-0-15ml + 45 ml water after food 4. Cap GT 2 cap at bed time ( reduces inflmmation)

✅Exercise & Movement Guidance for You

You are doing flexibility exercises, which is very good 👍 You may gently include some of the next: –Quadriceps strengthening –Straight leg raises –Walking with support (10–15 min, twice a day)

❌ Do not: Squat Cross-legged sitting Too many stairs climbing Stop any movement that causes you sharp pain.

✅ DIET AND WEIGHT MANAGEMENT (Very Crucial for You)

Any additional kilogram will increase the pressure on the knee joint by 4 times. Even a weight loss of 5–7 kg will be able to drastically lessen the knee strain.

✅ Include Warm, freshly cooked food Moong dal, rice, millet Vegetables which are cooked (lauki, turai, carrot, pumpkin) Ghee (small quantity)

❌ Avoid Bakery products Fried snacks Cold foods Curd at night Excess sugar and refined flour

✅ Mental & Emotional Support

You have experienced: Cancer Surgery Long-term joint disease Living alone

All of these increase Vata and emotional stress, which in turn also exacerbate joint degeneration.

On a daily basis: 10 minutes of Anulom Vilom Positive routine Sun exposure in the morning

✅ WHEN IS SURGERY TRUELY NECESSARY?

Surgery to replace the knee should be strongly considered only when: –Pain is very severe even at rest –Walking, even inside the house, becomes impossible –Legs become severely deformed –Sleep is disturbed every day because of pain

👉 Judging from what you have told me, you are not at that stage yet.

😊 Final Honest Guidance for You

Especially since: You are living alone You are not emotionally prepared for surgery The pain is still manageable It is absolutely sensible of you to seek a non-surgical alternative.

With right Ayurvedic treatments, proper medication, weight loss and joint-protective lifestyle you will be able to:

👉 Postpone knee replacement for many years 👉 Improve walking capacity 👉 Keep your independence 👉 Be able to live comfortably without worry of surgery for the time being

If you want, you can send me: Your most recent X-ray/MRI report Your current weight & height Any drugs that you are taking presently

So I’m able to assist you in a more accurate way.

Good health to you!😊

Warm Regards Dr Snehal ‍‌‍‍‌‍‌‍‍‌Vidhate

Given the situation you’ve outlined, there are some Ayurvedic approaches that could potentially support knee health and manage osteoarthritis symptoms. However, since surgery was recommended, please ensure that any alternative measures taken are closely monitored and approved by a healthcare professional.

Focusing on managing your weight is crucial, as excess body weight can significantly strain your knees. Begin with dietary changes tailored to your constitution (Prakriti). Consuming a light, easily digestable diet can help manage weight and improve joint health. Reduce intake of heavy, oily, or fried foods, which are not supportive of Vata balance or weight management.

Include more warm, cooked meals. Soups and stews made with mung dal, vegetables, and spices like ginger, turmeric, and cumin can be very nourishing. Incorporate ghee in your diet, which can improve joint lubrication. Eating smaller, more frequent meals can also help sustain energy without overwhelming digestion.

Yoga and gentle exercises can also be a great support to your regular knee exercises. Try practicing yoga postures like Tadasana (Mountain Pose), Virabhadrasana (Warrior Pose), and Setu Bandhasana (Bridge Pose) which can strengthen the muscles around the knee and increase flexibility without overstraining the joints.

Topical application of medicated oils or balms like Mahanarayan oil or Dhanvantaram thailam can provide additional relief from stiffness. Warm oil massages followed by hot fomentation can reduce pain and support mobility. Ensure you move each joint through its full range of motion daily, but avoid painful exercises.

Consider herbs like Ashwagandha and Shallaki (Boswellia serrata) for support. These are thought to have properties that help reduce inflammation and improve joint health. Ayurvedic supplements, should be taken under the guidance of a trained practitioner.

Prior to implementing any of these suggestions, please consult your healthcare provider to ensure they’re appropriate given your medical history and current condition. Remember, the management of osteoarthritis and weight loss requires a long-term commitment and should be approached gently and with patience.

Considering your osteoporosis and reluctance towards knee replacement, the Siddha-Ayurvedic approach can offer supportive methods to possibly ease your condition. It’s important to manage your vata dosha, which is likely aggravated, considering joint issues and previous medical history.

Firstly, focus on a vata-pacifying diet. This means warm, unctuous foods that are easy to digest. Incorporate ghee, sesame oil, and spices like ginger and turmeric - daily. They have anti-inflammatory and analgesic properties and can enhance agni. Try to avoid cold and raw foods which may aggravate vata further.

Herbal formulations may provide support. Consider using Ashwagandha and Shallaki (Boswellia) known for their bone-strengthening and anti-inflammatory effects. Consult with a qualified practitioner for proper dosages, particularly with your medical history.

Massage with warm sesame oil can be beneficial. Apply it to the joints in a gentle circular motion. This can improve circulation and reduce stiffness. Do it daily before a warm bath or shower.

Continue light exercise but avoid any that cause pain or discomfort. Gentle yoga or tai chi could be beneficial to maintain flexibility without overstraining the knee.

Lastly, weight management is crucial. Excess weight puts added pressure on your knees, worsening the situation. Even a small reduction can make a difference, enhancing your mobility and reducing stress on the joints.

However, since surgery has been recommended, closely discussing your options with a healthcare professional is vital. If pain or mobility worsens severely, ensure you seek timely medical attention.
