Patient Report: Lumbar Spine & Symptoms Patient Name: جی جمیل مینگل Date of Birth: 5 September 1985 Presenting Complaint: Chronic lower back pain with radiation to the right leg --- History of Present Illness (Pain Story) The patient reports a history of low back pain for over 1 year, which has progressively worsened. The pain is localized primarily at the lower lumbar region (L4-S1) and radiates down the right leg, following the path of the sciatic nerve. Key symptoms include: Sharp, shooting pain in the right leg, especially when sitting or bending forward. Occasional tingling and numbness in the right foot. Pain aggravated by lifting, twisting, or prolonged sitting. Temporary relief when lying on the floor with knees slightly bent or when placing a small rolled towel under the lower back. Episodes of stiffness and limited range of motion in the lower back. Reduced sexual function due to chronic pain and discomfort. The patient denies any recent trauma or systemic symptoms such as fever or weight loss. --- MRI Findings (Lumbar Spine) Imaging Date: 22-Apr-2025 (latest lumbar MRI available) Findings: 1. Lumbar Spondylosis: Mild degenerative changes noted throughout the lumbar spine. 2. Disc Desiccation: At L4-L5 and L5-S1, indicating loss of disc hydration. 3. Disc Bulge: Mild to moderate bulge at L4-L5. 4. L5-S1 Disc: Mild to moderate right-sided disc bulge causing right L5-S1 nerve root compression. 5. Spinal Canal: Mild central canal stenosis at L5-S1. 6. Alignment: Mild straightening of lumbar lordosis. 7. Other Observations: No evidence of fracture, mass, or acute disc herniation. Impression: Lumbar spondylosis with disc bulges at L4-L5 and L5-S1. Right L5-S1 nerve root compression, correlating with right leg radicular pain. Mild lumbar lordosis straightening and mild canal stenosis. --- Clinical Correlation The MRI findings are consistent with the patient’s symptoms of low back pain radiating to the right leg, likely secondary to nerve root compression at L5-S1. Conservative management, including physical therapy, nerve pain medications, and posture modification, is currently recommended. Current Medications: Tizanidine Pregabalin 75 mg (discontinued previously) Lacosamide (recently started) Omeprazole Cobalamin (Vitamin B12) Piroxicam Betacyclodextrin Additional Notes: Patient reports better symptom control with Lacosamide compared to Pregabalin. Use of a small roll towel under the lower back while lying down provides temporary relief. Patient has limited ability to perform sexual activity due to pain and nerve۔

You are taking: 1.Tizanidine – muscle relaxant 2. Lacosamide – nerve stabilizer (you feel better on this ✅) 3.Omeprazole – stomach protector 4.Vitamin B12 – nerve nourishment 5. Piroxicam Betacyclodextrin – pain killer ❌ Pregabalin – stopped (since Lacosamide works better)

➡️ The first piece of good news is that these are the right medicines for controlling nerve pain and spasm.

However, long-term use of painkillers and muscle relaxants can weaken the digestive system and, according to Vata, make it worse, so Ayurvedic support is very necessary at this time.

✅ Ayurvedic Treatment Plan

PANCHAKARMA (Purification - only under the supervision) In case pain becomes chronic or increases: Matra Basti (oil enema) with: Bala Taila This nourishes the nerves and lumbar spine directly.

INTERNAL MEDICATION

You are allowed to take the following along with your present medicines (it would be better if there is a gap of 1–2 hours between them):

1.Yograj Guggulu - 1 tablet twice daily after food (Reduces nerve compression & stiffness)

2. Dashmoolarishta – 20 ml with equal water at night (Best anti-Vata spine tonic)

3. Maharasnadi Kashayam – 15 ml twice daily with an equal amount of water before food (Perfectly suited for nerve and joint inflammation)

4. Ashwagandha Churna - ½ tsp at night with warm milk (Nerve, muscle strength & stamina improvement)

5. Balarishta – 15 ml at night (Improves pelvic, lower back & sexual strength)

✅ External Ayurvedic Therapy (Very Important)

Daily massage of: - Lower back - Hip region - Back of thighs with: Mahanarayan Taila

Medicines 1 Maha Rasnadi Kashayam: 15ml mixed with 45ml warm water, twice daily before food. 2 Trayodashang Guggulu: 2 tablets twice daily with warm water. 3 Ksheerabala 101 (Softgel Capsules): 2 capsules daily (1 morning, 1 night) with warm milk. 4 Ashwagandha Churna: 1 teaspoon mixed with 5 Kapikacchu Churna (1/2 teaspoon) in warm milk at bedtime.

External Therapy Local Abhyanga: Apply warm Mahanarayan Thailam gently on the lower back and down the right leg. Leave for 30 minutes before warm water bath. Kati Pichu: Soak a cotton pad in warm Murivenna Oil or Mahanarayan Oil and place it on the lower back (L4-S1 area) for 20 minutes daily while lying down.

Dietary Advice Vata Pacifying Diet: Eat warm, cooked, and slightly oily foods (using Ghee). Exclusions: Strictly avoid chickpeas (chana), kidney beans (rajma), potatoes, cauliflower, and cold drinks. Hydration: Drink only warm water to prevent constipation.
