From the symptoms which you have mentioned, eventhough you have underwent twice virechana, temporarily pitta will reduce but since your Agni is weak body keeps generating ama , hence no much improvement is seen Until Agni is rekindled panchakarma will not work properly So at present stop all panchakarma/ detox therapies focus on rekindling Agni and clearing ama Sithophaladi churna + Talisadi churna + -1/4 th tsp with honey Mulethi kwath 1/2 tsp with warm water Steam inhalation Chitrakadi vati - to be chewed Drink warm turmeric milk Drink warm water throughout the day

Shortness of breath with the history of chest infection, especially involving the hila - region could require urgent medical attention. Given the respiratory involvement, you must consult with a healthcare provider as this could be a sign of severe or progressive condition. It’s crucial to prioritize respiratory stability and seek medical evaluation for a precise diagnosis, ensuring no immediate danger to life.

In terms of Ayurveda, considering the complexities of your condition and current symptoms like mand agni, you will require a very personalized approach. Shortness of breath and your other symptoms like liver disorder, hypothyroidism, and low ferritin suggest a need to balance Kapha and Pitta doshas and nourish Dhatus particularly Rasa and Rakta.

Firstly, enhancing agni (digestive fire) is essential. Try sipping warm ginger water throughout the day or consuming a small piece of ginger with a bit of rock salt before meals. Avoid heavy, cold, or greasy foods, as these can further imbalance Kapha. Focus on easily digestible meals and include foods like moong dal khichdi, stewed apples, and ginger tea, which are light and warm.

Having done Virechana, ensuring adequate rest post-detoxification is important. To support liver function and hemoglobin, include workout like light jogging or brisk walking, breathing exercises, and yoga practices like Kapalbhati. Consider Ayurvedic medications such as Punarnava or Triphala as adjuncts but always consult an Ayurvedic practitioner for correct dosage and coordination with allopathic treatment.

Address hypothyroidism by considering Ashwagandha or Guggulu-based formulations, as these may help support thyroid function. Similarly, hairfall might benefit from Amla supplementation, known for rejuvenating properties.

Address the vertigo and headache with regular routine of Abhyanga (oil massage) using sesame oil, and Nasya with Anu taila—again under guidance. Stay consistent with your practices and work closely with a trusted Ayurvedic practitioner and modern medicine doctor who can adjust your regimen as necessary. Always match naturopathic efforts with regular monitoring by a healthcare professional.
